摘  要:目的探讨结直肠癌组织的Ⅰ型胶原蛋白α1链(COL1A1)和α2链(COL1A2)水平及临床意义。方法采用GEPIA分析来自TCGA数据库的367例结直肠癌(275例结肠癌+92例直肠癌)的COL1A1、COL1A2水平,实时定量PCR检测2016年1月至2018年12月手术切除的106例结直肠癌组织和93例癌旁组织的COL1A1、COL1A2水平,采用受试者工作特征(ROC)曲线评估组织两者水平诊断结直肠癌的效能,分析组织两者水平与结直肠癌临床病理特征的关系,采用GEPIA分析两者水平与结直肠癌总生存期(OS)和无病生存期(DFS)的关系。结果GEPIA在线分析显示,275例结肠癌组织的COL1A1和COL1A2水平均高于对应41例正常组织,而92例直肠癌组织中仅COL1A1水平高于10例正常组织(P<0.05)。106例结直肠癌组织的COL1A1和COL1A2水平分别为4.013±1.705和3.535±1.354,高于93例癌旁组织的1.544±0.678和1.613±0.741(P<0.05),且COL1A1和COL1A2水平呈正相关(r=0.518,P<0.001)。ROC曲线显示组织COL1A1和COL1A2水平诊断结直肠癌的曲线下面积分别为0.894(95%CI:0.848~0.941)和0.882(95%CI:0.837~0.927)。两者水平均与淋巴结转移、肿瘤直径、TNM分期和T分期有关(P<0.05)。COL1A1水平仅与DFS有关,而COL1A2水平与OS和DFS均有关,其中高水平者的OS和DFS较差。结论COL1A1和COL1A2在结直肠癌中高表达,在该肿瘤的诊断、病情预测和预后评估上有一定价值,两者共同促进了结直肠癌的发生发展。Objective To investigate the levels and clinical significance of collagen type I alpha 1 chain(COL1A1)and alpha 2 chain(COL1A2)in colorectal cancer tissues.Methods GEPIA was used to analyze the COL1A1 and COL1A2 levels of 367 colorectal cancer tissues including 275 cases of colon cancer and 92 cases of rectal cancer from TCGA database.Real-time quantitative PCR was used to detect the levels of COL1A1 and COL1A2 in 106 colorectal cancer tissues and 93 adjacent tissues surgically removed from January 2016 to December 2018.Receiver operating characteristic(ROC)curve was used to evaluate the tissue levels of two factors for diagnosing colorectal cancer.The relationship between tissue levels of two factors and clinicopathological characteristics of colorectal cancer was analyzed.GEPIA was applied to analyze the relationship between tissue levels of two factors and prognosis including overall survival(OS)and disease-free survival(DFS).Results GEPIA online analysis showed that both levels of COL1A1 and COL1A2 in 275 cases of colon cancer tissues were higher than those in 41 cases of normal tissues,while only COL1A1 level in 92 cases of rectal cancer tissues was higher than that in 10 cases of normal tissues(P<0.05).Levels of COL1A1 and COL1A2 in 106 colorectal cancer tissues were 4.013±1.705 and 3.535±1.354,higher than 1.544±0.678 and 1.613±0.741 in 93 adjacent tissues(P<0.05).A positive correlation was observed between COL1A1 and COL1A2(r=0.518,P<0.001).ROC curve showed that the area under the curve was 0.894(95%CI:0.848-0.941)and 0.882(95%CI:0.837-0.927),respectively.Both levels were related to lymph node metastasis,tumor diameter,TNM stage and T stage(P<0.05).The COL1A1 level was only related to DFS,while COL1A2 level was related to OS and DFS,and the OS and DFS of the high level were poor.Conclusion COL1A1 and COL1A2 are highly expressed in colorectal cancer,which have certain value in the diagnosis,disease prediction and prognosis evaluation of the tumor.Both of them promote the occurrence and development of co
