淡漠评定量表-照顾者版在养老院阿尔茨海默病患者中的信效度研究

摘　　要：目的评估淡漠评定量表-照顾者版(AES-I)在养老院阿尔茨海默病(AD)患者中的信效度。方法 2018年6-8月使用AES-I、神经精神科问卷-淡漠分量表(NPI-A)和神经精神科问卷-抑郁分量表(NPI-D)对127例AD患者进行调查分析。结果 AES-I总的Cronbach′sα系数为0.977,各维度α系数为0.860~0.954;总的折半信度为0.967,各维度折半信度为0.861~0.958;总的再测信度系数为0.927,各维度再测信度系数为0.760~0.841。AES-I与NPI-A得分呈高度相关(r=0.839,P<0.001),聚合效度较好;AES-I与NPI-D得分不相关(P=0.471),有较好的区分效度;探索性因子分析提取出认知-行为、情感2个因子,累积方差贡献率78.13%。以NPI-A为金标准绘制ROC曲线,ROC曲线下面积为0.922(P<0.001),AES-I选取42.5分为界值时,对应量表的敏感度、特异度较高,分别为0.973和0.687。结论 AES-I信效度良好,在养老机构AD患者中最佳淡漠划界分值为42.5分,可用于养老院AD患者淡漠症状测评。Objective To evaluate the reliability and validity of the Apathy evaluation scale-informant version(AES-I) among patients with Alzheimer’s disease in nursing homes. Method Data were collected from 290 Alzheimer’s patients by the AES-I,neuropsychiatric inventory-apathy sub-scale(NPI-A),neuropsychiatric inventory depression sub-scale(NPI-D). Result The Cronbach’s α was 0.977 for the total scale and 0.860~0.954 for each dimensions. The split-half reliability was 0.967 for the total scale and 0.861~0.958 for each dimensions. The test-retest reliability was 0.927 for the total scale and 0.760~0.841 for each dimensions. The AES-I scores were correlated with NPI-A score(r=0.839,P<0.001),showing good convergent validity. The AES-I score and NPI-D score were not correlated(P=0.471),showing good discriminant validity. Exploratory factor analysis extracted two common factors,which explained 78.13% of the total variance. The optimal critical value for AES-I was 42.5 scores. The sensitivity and specificity were 0.973 and 0.687 respectively, the areas under the curve was 0.922(P<0.001). Conclusion Chinese version of AES-I is reliable and valid, and is suitable for measuring apathy among Alzheimer’s in nursing homes. The optimal critical value of AES-I for the Alzheimer’s patients is 42.5 scores.
