摘　　要：目的探讨Ⅱ型多磷酸肌醇4-磷酸酶(INPP4B)蛋白在胃腺癌中表达情况及其对患者手术预后的影响。方法采用免疫组织化学染色(IHC)S-P三步法检测平均随访5年的140例胃腺癌患者(2011年7月至2016年7月在该院行首次胃癌手术)的癌组织及相应的癌旁组织(肉眼观察距离癌组织边缘大于5cm)中INPP4B蛋白表达情况。通过SPSS17.0统计学软件分析胃腺癌组织中INPP4B蛋白的表达情况与胃腺癌患者临床病理结果之间的关系;运用生存分析Kaplan-Meier法分析其对于患者手术预后的影响。结果IHC结果显示,INPP4B蛋白在胃腺癌组织、癌旁组织中表达阳性率分别为21.43%、83.57%,前者小于后者,差异有统计学意义(χ^2=1.084,P<0.01)。INPP4B蛋白在胃腺癌组织中的表达水平与淋巴结转移、分化程度、肿瘤TNM分期呈负相关(P<0.01),而与性别、年龄、肿瘤大小无关(P>0.05)。生存分析Kaplan-Meier法显示,INPP4B高表达组较低表达组的生存期明显延长,无进展生存期及总生存期均延长,差异均有统计学意义(P<0.01)。结论INPP4B蛋白在胃腺癌组织中均表达下调,可能是胃腺癌的一种抑制因子,与胃腺癌的分化程度、淋巴结转移及TNM分期呈负相关,可作为胃腺癌患者预后评估的重要指标。Objective To investigate the expression of Inositol polyphosphate-4-phosphatase,typeⅡ(INPP4B)protein in gastric adenocarcinoma and its effect on the prognosis of patients.Methods Immunohistochemical(IHC)staining of S-P in three steps was used to detect the expression of INPP4B protein in the cancer tissues and the corresponding adjacent tissues(the distance to the margin of the carcinoma was greater than 5 cm)of 140 patients with gastric adenocarcinoma(underwent the first gastric cancer surgery in the hospital from July 2011 to July 2016).The relationship between the expression of INPP4B protein in gastric adenocarcinoma and the clinico pathological results of patients with gastric adenocarcinoma was analyzed by SPSS17.0 software.The survival analysis Kaplan-Meier method was used to analyze the effect of INPP4B on the prognosis of patients.Results IHC showed that the positive rate of INPP4B protein expression in gastric adenocarcinoma tissues(21.43%)was positive in adjacent tissues(83.57%),the former was less than the latter,the difference was statistically significant(χ^2=1.084,P<0.01).The expression levels of INPP4B protein in gastric adenocarcinoma tissues were negatively correlated with lymph node metastasis,differentiation degree and tumor TNM stage(P<0.01),but not related to gender,age and tumor size(P>0.05).Survival analysis Kaplan-Meier method showed that the survival time of INPP4B high expression group was significantly longer than that of low expression group,progression-free survival and overall survival were prolonged,and the differences were statistically significant(P<0.01).Conclusion The expression of INPP4B protein is down-regulated in gastric adenocarcinoma,which may be an inhibitory gene of gastric adenocarcinoma.It is closely related to the differentiation of gastric adenocarcinoma,lymph node metastasis and TNM staging.It can be used as an important indicator for prognosis evaluation of patients with gastric adenocarcinoma.
