基质金属蛋白酶7和基质金属蛋白酶9与甲状腺微小癌临床病理特征的相关性研究

摘　　要：目的研究甲状腺微小癌(papillary thyroid microcarcinoma,PTMC)的MCM-7、MMP-9表达与临床病理特征之间的关系,为临床提供简便有效的预后判断。方法收集32例确诊PTMC病例,分别取肿瘤组织标本作为实验组,取其癌灶以外的甲状腺组织标本作为对照组,通过免疫组化方法测定两组标本的MCM-7、MMP-9表达,统计学分析实验组与对照组之间的表达差异,并研究MCM-7、MMP-9表达与PTMC临床病理特征之间的相关性,判断其表达能否体现PTMC的侵袭性及预后。结果 PTMC实验组的MCM-7、MMP-9表达高于对照组,两组样本平均光密度值(average optical density,AOD)差异有统计学意义(P<0.05);PTMC实验组的MCM-7表达与肿瘤直径≥5 mm呈正相关(r_s=0.526,P=0.002),MMP-9表达与淋巴结转移呈正相关(r_s=0.411,P=0.020),MCM-7、MMP-9联合表达与临床病理特征的相关性无显著统计学差异。结论MCM-7、MMP-9在非肿瘤甲状腺组织标本中低表达,在PTMC肿瘤组织中高表达,MCM-7表达与肿瘤直径≥5 mm正相关,MMP-9表达与淋巴结转移正相关,可以作为PTMC辅助术前诊断、风险分层、预测预后的潜在指标。OBJECTIVE To study the relationship between the expression of MCM-7,MMP-9 and clinicopathological characteristics of papillary thyroid microcarcinoma(PTMC),so as to provide a simple and effective prognosis.METHODS 32 cases of PTMC were collected.The tumor tissue samples were taken as the experimental group.The thyroid tissue samples outside the tumor focus were taken as the control group.The expression of MCM-7 and MMP-9 in the two groups were measured by immunohistochemistry.The difference between the experimental group and the control group was analyzed by statistics.The correlation between the expression of MCM-7 and MMP-9 and the clinicopathological characteristics of PTMC was studied,to determine whether the expression of PTMC can reflect the invasion and prognosis of PTMC.RESULTS The expression of MCM-7 and MMP-9 in PTMC group was significantly higher than that in control group(P<0.05).The difference of average optical density between the two groups was statistically significant(P<0.05);In PTMC group,the expression of MCM-7 was positively correlated with tumor diameter≥5 mm(Spearman r_s=0.526,P=0.002),and the expression of MMP-9 was positively correlated with lymph node metastasis(Spearman r_s=0.411,P=0.020);There was no significant correlation between the expression of MCM-7 and MMP-9 and the clinicopathological characteristics.CONCLUSIONThe expression of MCM-7 and MMP-9 is low in non tumor thyroid tissues,high in PTMC tumor tissues,positive correlation between MCM-7 expression and tumor diameter≥5 mm,positive correlation between MMP-9 expression and lymph node metastasis,which can be used as potential indicators of PTMC assisted preoperative diagnosis,risk stratification and prognosis prediction.
