摘 要:目的:探讨血必净注射液对重症急性胰腺炎(SAP)大鼠腹内压的影响。方法:将15只SD大鼠随机分为对照组、SAP组和治疗组,每组各5只。SAP组和治疗组经胆胰管逆行注射4.5%牛磺胆酸钠溶液制备SAP模型,对照组仅予胆胰管逆行注射等量0.9%氯化钠溶液,治疗组造模后即予腹腔注射血必净注射液。3组大鼠造模24 h后观察并记录大鼠一般情况、腹内压、腹水量、胰腺及小肠病理改变,检测血浆血清淀粉酶(AMS)、内毒素(ET)、炎性细胞因子(TNF-α、IL-1β、IL-6)和二胺氧化酶(DAO)水平。结果:与对照组比较,SAP组IAP、腹水量、胰腺和小肠病理评分,血浆AMS、IL-1β、IL-6、ET和DAO水平均明显升高(P<0.05);治疗组胰腺和小肠病理评分,血浆AMS、TNF-α、IL-6和DAO水平均明显升高(P<0.05);与SAP组比较,治疗组IAP、腹水量、胰腺和小肠病理评分,血浆AMS、TNF-α、IL-1β、IL-6、ET和DAO水平均明显下降(P<0.05)。结论:血必净注射液可降低SAP大鼠IAP,减少腹水量及降低血浆炎性细胞因子水平,延缓肠黏膜屏障功能障碍的发生。Objective:To investigate the effect of Xuebijing injection on intra-abdominal pressure(IAP)in rats with severe acute pancreatitis(SAP).Methods:Fifteen Sprague Dawley(SD)rats were randomly divided into three groups:control group(n=5),SAP group(n=5)and treatment group(n=5).SAP model was induced by retrograde injection of 4.5%sodium taurocholate through the biliopancreatic duct in SAP group and treatment group,while only 0.9%sudium chloride solution was injected in control group.The treatment group received transperitoneal use of injectable Xuebijing immediately after SAP modeling.After 24 hours of successful modeling,the three groups were recorded for intra-abdominal pressure(IAP),ascites volume(AS),pathological changes of pancreas and small intestine,and measured for plasma levels of amylase(AMS),endotoxin(ET),inflammatory cytokines(TNF-α,IL-1β and IL-6)and diamine oxidase(DAO).Results:Compared with the control group,the SAP group presented significantly increased IAP,AS,pathological scores of pancreas and small intestinal,as well as higher plasma levels of AMS,IL-1β,IL-6,ET,and DAO(P<0.05);the treatment group also showed increased pathological scores of pancreas and small intestinal and higher plasma levels of AMS,TNF-α,IL-6,and DAO(P<0.05).Compared with SAP group,the treatment group presented significantly decreased IAP,AS,pathological scores of pancreas and small intestinal,and lower plasma levels of AMS,TNF-α,IL-1β,IL-6 ET,and DAO(P<0.05).Conclusion:In rats with severe acute pancreatitis,Xuebijing injection may reduce IAP,AS and plasma inflammatory cytokines,and also delay the development of gut mucosal barrier dysfunction.
