摘　　要：目的:探讨血清C反应蛋白、降钙素原及血淀粉酶对重症急性胰腺炎的早期临床预测价值。方法:选取2018年2月—2019年4月我院收治的急性胰腺炎患者38例,根据病情轻重程度、APACHEⅡ评分将其分为重症急性胰腺炎(SAP)组17例和轻症急性胰腺炎(MAP)组21例。选择同期体检为正常的38例对象为健康对照组。分析各组血清C反应蛋白、降钙素原及血淀粉酶指标与SAP的相关性,评价对SAP诊断及病变严重程度评估的应用价值。结果:SAP组入院时、入院2d、入院3d时测得的PCT、CRP、血淀粉酶水平均高于M A P组和对照组(P<0.01);SAP组、MAP组血淀粉酶水平随时间推移呈下降趋势(P<0.05);PCT/CRP联合检测的灵敏度高于单独检测(P<0.01)。结论:PCT、CRP、血淀粉酶三者检测对SAP早期具有一定的临床预测价值,同时通过提高诊断的准确性,有利于改善SAP患者的预后,提高患者生活质量,且操作简单、花费低。Objective To investigate the early clinical predictive value of serum C-reactive protein,procalcitonin and blood amylase for severe acute pancreatitis.Methods 38 patients with acute pancreatitis treated in our hospital from February 2018 to April 2019 were selected and divided into 17 cases of severe acute pancreatitis(SAP)and 21 cases with mild acute pancreatitis(MAP)according to their severity and APACHEⅡscore.38 subjects with normal physical examination during the same period were selected as the healthy control group.The correlations of serum C-reactive protein,procalcitonin,and blood amylase indexes with SAP were analyzed in each group,and their application value in the diagnosis of SAP and assessment of the severity of lesions were evaluated.Results The levels of PCT,CRP,and blood amylase measured in the SAP group at admission,2 days,and 3 days after admission were higher than those in the MAP group and the control group(P<0.01);the levels of blood amylase in the SAP group and MAP group decreased over time Trend(P<0.05);the sensitivity of the combined PCT/CRP test was higher than that of the single test(P<0.01).Conclusion The detection of PCT,CRP,and blood amylase has certain clinical predictive value in the early stage of SAP.At the same time,by improving the accuracy of diagnosis,it is beneficial to improve the prognosis of SAP patients,improve the quality of life of patients,and is simple and low-cost.
