从“治风先治血,血行风自灭”论治行痹述要

摘　　要：通过阅读经典著作结合自身临床体会及典型病案,对“行痹”的发病特点及“治风先治血,血行风自灭”的治疗机制进行了整理、归纳及探讨,以期对“治风先治血,血行风自灭”治疗行痹的病机形成系统认知。风为百病之长,行痹发病以风邪为主,由外因和内因共同致病。风与血密切相关,风致血病,血治风治。“治风先治血,血行风自灭”涵义有三:一为调理血促使体内之风祛除;二为制约风药的燥烈;三为通过治血使集体无法生成内风,同时外风难袭。历代医家治疗行痹时广泛应用该理论,但各有特点。该理论适用于行痹治疗,亦可于多种痹证的治疗中应用,包括善后调理。治疗行痹时,须根据病机,治风时首先治血,但不可仅仅治血,常多法合用进而达到血行风灭的目的。This article mainly through reading the classic works combined with his own clinical experience and typical medical records,the incidence of“Xingbi”characteristics and“Treating Blood First Before Treating Wind,Extinguishing Wind by Blood Circulation”treatment mechanism are sorted out,summarized and discussed.In the hope of forming a systematic cognition of the pathogenesis of“treating blood first,curing blood and curing wind by itself”in treating“Xingbi”.Wind is the chief cause of all diseases.Wind-evil is the main cause of bi-movement,which is caused by external and internal factors.Wind and blood is closely related,wind-induced hemopathy,blood treatment wind.There are three meanings in“treating wind first to cure blood,blood circulation to kill itself”:one is to regulate blood to promote the elimination of wind in the body;two is to restrict the dryness and fierceness of wind medicine;three is to make the collective unable to generate internal wind through treating blood,while the external wind is difficult to attack.This theory has been widely used in the treatment of Arthralgia in past dynasties,but each has its own characteristics.This theory can be applied to the treatment of Arthralgia,and can also be applied to various arthralgia syndromes,including the treatment after Arthralgia.When treating Arthralgia,we should treat blood first according to the pathogenesis and wind,but not just blood,often combined with multiple methods to achieve the purpose of extinction.
