摘　　要：目的探讨伴肾功能异常的多发性骨髓瘤的临床特征。方法选择初诊的多发性骨髓瘤患者122例,包括肾功能正常组60例与肾功能异常组60例。所有患者都给予血液学与骨髓穿刺病理学检查。结果肾功能异常组的血清CysC值显著高于肾功能正常组(P<0.05);肾功能异常组的临床分期、Ig蛋白类型、轻链类型与肾功能正常组对比差异有统计学意义(P<0.05);肾功能异常组的CD38、CD138表达阳性率显著高于肾功能正常组(P<0.05)。在122例患者中,Spearman检验显示肾功能异常与CD38、CD138表达阳性率和CysC、临床分期、Ig蛋白类型、轻链类型都显著相关(P<0.05)。结论伴肾功能异常的多发性骨髓瘤比较常见,血液学检查、骨髓穿刺检查特征与患者的肾功能异常显著相关。Objective To investigate the clinical features of multiple myeloma with renal dysfunction.Methods 122 patients with multiple myeloma who were newly diagnosed were selected that included 60 patients with normal renal function and 60 patients with renal dysfunction.All patients were given hematology and bone marrow puncture pathology.Results The serum CysC value of the renal dysfunction group were significantly higher than that of the normal renal function group(P<0.05).The clinical stage,Ig protein type,light chain type and normal renal function of the renal dysfunction group were statistically significant(P<0.05).The positive rate of CD38 and CD138 expression in the renal dysfunction group were significantly higher than that of the normal renal function group(P<0.05).In 122 patients,Spearman's test showed there were significant correlation between renal dysfunction and CD38,CD138 expression and CysC,clinical stage,Ig protein type,and light chain type(P<0.05).Conclusion Multiple myeloma with renal dysfunction is more common.The characteristics of hematology and bone marrow examination are significantly correlated with renal dysfunction.
