内镜黏膜下剥离术治疗早期胃癌的效果分析

摘　　要：目的探讨内镜黏膜下剥离术(ESD)治疗早期胃癌的效果。方法回顾性选择早期胃癌患者60例,根据治疗方法分为观察组和对照组,各30例。观察组给予ESD治疗,对照组给予外科手术治疗,记录与随访两组预后。结果所有患者都完成治疗,无术中输血病例,观察组的手术时间、术中出血量与术后住院时间都显著短于(少于)对照组(P<0.05);观察组术后7 d的出血、穿孔、感染等并发症发生率为3.33%,显著低于对照组的23.33%(P<0.05)。观察组术后1 d、3 d与7 d的VAS疼痛评分都显著低于对照组(P<0.05)。随访至今,观察组有2例复发,复发率为6.67%(2/30),对照组无患者复发,组间比较差无统计学差异(t=2.069,P=0.492,P>0.05)。结论相对常规外科手术,ESD治疗早期胃癌能减少对患者的创伤,减少并发症的发生,促进患者康复与缓解疼痛。Objective To investigate the effects of endoscopic submucosal dissection(ESD)on early gastric cancer.Methods 60 patients with early gastric cancer were retrospectively selected and divided into the observation group and the control group according to the treatment method,30 cases each.The observation group were treated with ESD,the control group were given conventional surgical treatment,and the prognosis of the 2 groups were recorded and followed up.Results All patients were completed the treatment without intraoperative blood transfusion.The operation time,intraoperative blood loss,and postoperative hospital stay in the observation group were significantly shorter(less than)the control group(P<0.05).The incidence of complications such as bleeding,perforation,and infection 7 days after operation in the observation group was 3.33%,which was significantly lower than 23.33%in the control group(P<0.05).The VAS pain scores in the observation group at 1 d,3 d,and 7 d after surgery were significantly lower than those of the control group(P<0.05).After the followed-up,there was 2 cases of recurrence in the observation group,with a recurrence rate of 6.67%(2/30).There was no recurrence in the control group.There was no significant difference between the 2 groups(t=2.069,P=0.492,P>0.05).Conclusion Compares with conventional surgery,ESD treatment of early gastric cancer could reduce trauma to patients,reduce the occurrence of complications,promote rehabilitation and pain relief.
