血清CA199、CA125、HE4联合检测在卵巢癌诊断中的应用价值

摘　　要：目的探讨血清糖类抗原199(CA199)、糖类抗原125(CA125)、人附睾蛋白4(HE4)联合检测在卵巢癌诊断中的应用价值。方法选取228例卵巢癌患者作为研究组,另有同期的195例卵巢良性病变患者为对照1组,匹配同期体检的228例健康女性作为对照2组。检测并比较3组的血清CA125、CA199、HE4水平及其阳性率、各指标及联合检测对卵巢癌的诊断效率、卵巢癌不同临床分期CA125、CA199、HE4水平。结果经方差分析及LSD-t检验,3组间CA125、CA199、HE4水平均有统计学差异,研究组>对照1组>对照2组(P<0.05)。经卡方检验,3组间的各指标的阳性率均不同,研究组各指标的阳性率均大于对照1组、对照2组,对照1组CA125、CA199阳性率大于对照2组(P<0.05)。经McNemar检验,各指标与联合检测相比,其敏感度均低于联合检测(P<0.05),但其特异度无明显统计学差异(P>0.05)。经独立样本t检验,卵巢癌Ⅰ~Ⅱ期的CA125、CA199、HE4水平显著低于Ⅲ~Ⅳ期(P<0.05)。结论卵巢癌患者血清CA125、CA199、HE4呈高表达,且其水平变化与卵巢癌的临床进展密切相关。CA125、CA199、HE4三者联合检测诊断效率较高,值得临床推广。Objective To analyze the application value of combined detection of serum carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125)and human epididymal protein 4(HE4)in the diagnosis of ovarian cancer.Methods 228 patients with ovarian cancer were selected as the research group,195 patients with benign ovarian lesions in the same period were selected as the control group 1,and 228 healthy women matched with physical examination in the same period were selected as the control group 2.The levels of serum CA125,CA199,and HE4 and their positive rates among the 3 groups,the diagnostic efficiency of each index and combined detection for ovarian cancer,and the levels of CA125,CA199,and HE4 in different clinical stages of ovarian cancer were detected and compared.Results Through variance analysis and LSD-t test,the levels of CA125,CA199 and HE4 among the 3 groups were statistically different,and the research group>the control group 1>the control group 2(P<0.05).Chi-square test showed that the positive rates of each index among the 3 groups were different.The positive rates of each index in the research group were higher than those of the control group 1 and control group 2.The positive rates of CA125 and CA199 in the control group 1 were higher than those of the control group 2(P<0.05).McNemar test showed that the sensitivity of each index was lower than that of the combined test(P<0.05),but the specificity had no significant difference(P>0.05).Independent sample t test showed that the levels of CA125,CA199 and HE4 in phaseⅠ~Ⅱwere significantly lower than those in phaseⅢ~Ⅳ(P<0.05).Conclusion The levels of serum CA125,CA199 and HE4 are highly expressed in patients with ovarian cancer,and their changes are closely related to the clinical progress of ovarian cancer.The combined detection and diagnosis of CA125,CA199,and HE4 has high diagnostic efficiency and is worthy of clinical promotion and research.
