结直肠癌患者癌组织CTEN、E-cadherin及Vimentin蛋白的表达水平及意义

摘　　要：目的探讨结直肠癌(CRC)患者癌组织中羧基末端张力蛋白样分子(CTEN)、上皮型钙黏素(E-cadherin)、波形蛋白(Vimentin)的表达水平及其意义。方法选取2015年11月至2017年12月在广西医科大学第四附属医院行CRC手术切除的存档蜡块和癌旁正常组织(距病变部位>5 cm处,且证实为无癌浸润)石蜡标本各90例作为研究对象。采用免疫组化法检测CRC组织中E-cadherin、CTEN、Vimentin的表达,卡方检验各蛋白与CRC患者临床病理参数的关系。结果E-cadherin在CRC组织中的阳性表达率低于癌旁组织,Vimentin、CTEN在CRC组织中的阳性表达率高于癌旁组织(P<0.05)。TNM分期T1-3期的CRC组织E-cadherin阳性表达率高于T4期,Vimentin、CTEN阳性表达率低于T4期;未合并淋巴结转移和神经侵犯的CRC组织中E-cadherin的阳性表达率高于合并淋巴结转移和神经侵犯的组织,Vimentin、CTEN的阳性表达率低于合并淋巴结转移和神经侵犯的组织;肿瘤直径<5 cm的CRC组织Vimentin、CTEN阳性表达率低于直径≥5 cm的组织;肿瘤分化成度低的CRC组织Vimentin的阳性表达率低于高分化肿瘤组织(P<0.05)。CRC组织中E-cadherin的表达与Vimentin、CTEN均呈显著负相关(r=-0.187、-0.339,P=0.016、0.002);CRC组织中Vimentin的表达与CTEN呈显著正相关(r=0.388,P=0.000)。结论E-cadherin、Vimentin、CTEN的阳性表达率与CRC的TNM分期、神经侵犯和淋巴结转移有关;其中Vimentin、CTEN阳性率与肿瘤大小相关,Vimentin阳性率与CRC的分化程度相关,三种蛋白的表达有一定相关性。Objective To explore the expression levels of C-terminal tensin-like protein(CTEN),epithelial cadherin(E-cadherin)and Vimentin in cancer tissues of colorectal cancer(CRC)patients and their significance.Methods During the period from November 2015 to December 2017,90 archived paraffin specimens and 90 normal para-cancerous tissues(more than 5 cm away from lesions confirmed with cancer-free)specimens from patients who underwent CRC excision in Fourth Affiliated Hospital of Guangxi Medical University were enrolled.The expression of E-cadherin,CTEN and Vimentin in CRC tissues was detected by immunohistochemistry.The relationship between all proteins and clinicopathological parameters of CRC patients was analyzed by chi-square test.Results The positive expression rate of E-cadherin in CRC tissues was lower than that in para-cancerous tissues,while positive expression rates of Vimentin and CTEN were higher than those in para-cancerous tissues(P<0.05).The positive expression rate of E-cadherin in CRC tissues with TNM staging at stage T1-3 was higher than that at stage T4,while positive expression rates of Vimentin and CTEN were lower than those at stage T4.The positive expression rate of E-cadherin in CRC tissue without lymph node metastasis and nerve invasion was higher than that with lymph node metastasis and nerve invasion,while positive expression rates of Vimentin and CTEN were lower than those with lymph node metastasis and nerve invasion.The positive expression rates of Vimentin and CTEN in CRC tissues with tumor diameter shorter than 5 cm were lower than those not shorter than 5 cm.The positive expression rate of Vimentin in CRC tissues with poor tumor differentiation was lower than that with high tumor differentiation(P<0.05).The expression of E-cadherin in CRC tissue was significantly negatively correlated with Vimentin and CTEN(r=-0.187,-0.339,P=0.016,0.002).The expression of Vimentin in CRC tissue was significantly positively correlated with CTEN(r=0.388,P=0.000).Conclusion The positive expression rates of E
