摘 要:目的探讨β-连接蛋白(β-catenin)在鉴别浸润性小叶癌和浸润性导管癌中的诊断意义。方法选取四川省简阳市人民医院2011年1月至2017年12月手术切除的乳腺癌病例51例为研究对象,患者均为女性,按浸润性癌类型不同分为浸润性导管癌(IDC)组32例,浸润性小叶癌(ILC)组11例,混合型癌(IDC+ILC)组8例,比较3组β-catenin表达的差异。结果在32例IDC和8例混合型癌的IDC成分中24例β-catenin呈连续强阳性表达于上皮细胞膜上(阳性率60.0%,24/40),16例异常表达于细胞核和质(阳性率40.0%,16/40),且E-cadherin和P120均为细胞膜表达阳性(阳性率100.0%,40/40);而在11例ILC和8例混合型癌的ILC成分中β-catenin表达明显缺失(阳性率0.0%,0/19),并伴随E-cadherin表达缺失(阳性率0.0%,0/19)及P120细胞质表达(阳性率100.0%,19/19),β-catenin在IDC与ILC中表达差异有统计学意义(P<0.05)。结论β-catenin在乳腺IDC和ILC中的不同表达模式在两种浸润性癌鉴别诊断中具有较高的应用价值。Objective To investigate the diagnostic significance ofβ-catenin in the differentiation between invasive lobular carcinoma and invasive ductal carcinoma.Methods A total of 51 cases with breast cancer treated with the surgical excision in The People's Hospital of Jianyang City from January of 2011 to December of 2017 were selected and all the patients were female.Those patients were divided into the invasive ductal carcinoma(IDC)group with 32 cases,invasive lobular carcinoma group with 11 cases,and mixed carcinoma group with 8 cases according to the types of invasive carcinoma.Theβ-catenin expression was compared between the three groups.Results In the IDC components of the 32 IDC cases and the 8 cases with mixed carcinoma,24 cases showed the continuously and strongly positive expression ofβ-catenin on epithelial cell membrane with the positive rate of 60.0%(24/40),16 cases showed abnormal expression ofβ-catenin in nucleus and cytoplasm with the positive rate 40.0%(16/40),and the expression of both E-cadherin and P120 was positive in cell membrane with the positive rate of 100.0%(40/40).In the ILC components of the 11 ILC cases and the 8 cases with mixed carcinoma,the expression ofβ-catenin was significantly absent with the positive rate of 0.0%(0/19),the expression of E-cadherin was also absent with the positive rate of 0.0%(0/19),and the cytoplasmic expression of P120 was positive with the positively rate of 100.0%(19/19).The expression ofβ-catenin in IDC and ILC was statistically different(P<0.05).Conclusion The different expression patterns ofβ-catenin between invasive ductal carcinoma and lobular carcinoma of breast have high application value in the differential diagnosis of the two types of invasive carcinoma.
