摘  要:目的:采用欧洲临床营养和代谢学会(ESPEN)制定的营养不良诊断标准评定患者自评主观全面评定(PG-SGA)和营养风险筛查(NRS2002)用于肿瘤住院患者的有效性。方法:选取2018年7~11月肿瘤住院患者,分别采用NRS2002和PG-SGA进行营养筛查或评定,然后将结果与ESPEN营养不良诊断标准进行比较。结果:共466例患者纳入本研究,根据ESPEN营养不良诊断标准,25.3%的患者存在营养不良。PG-SGA筛查结果为35.6%的患者营养不良,NRS2002筛查结果为43.1%的患者有营养风险。PG-SGA和NRS2002的灵敏度分别为94.9%和93.2%,特异度分别为84.5%和73.9%;与ESPEN营养不良诊断标准的相关性,kappa值分别为0.700和0.544,ROC曲线下面积分别为0.941和0.864。结论:PG-SGA和NRS2002两种筛查工具与ESPEN营养不良标准相关性均较好,灵敏度高,NRS2002更便捷省时,更方便肿瘤住院患者营养不良筛查。Objective:To verify the accuracy of two most commonly used screening tools by comparing their findings by the new ESPEN criteria.Methods:Nutritional screening was performed in 466 patients on admission to hospitals from July to November 2018 with two nutritional screening tools:Nutritional Risk Screening 2002(NRS2002)and Patient-generated Subjective Global Assessment(PG-SGA).The screening results were then compared to the new ESPEN diagnostic criteria for malnutrition.Results:According to the ESPEN diagnostic criteria,25.3%of the patients were classified as malnourished.According to the result of PG-SGA,35.6%of the patients were undernourished,and according to the result of NRS2002,43.1%of the patients were found to be at moderate/high risk of malnutrition.The sensitivity of PG-SGA and NRS2002 was 94.9%and 93.2%,and the specificity was 84.5%and 73.9%,respectively.PG-SGA was found to be better correlated with the ESPEN diagnostic criteria(κ=0.700,P<0.001)as compared to NRS2002(κ=0.544,P<0.001).ROC area under the curve of PG-SGA and NRS2002 was 0.941 and 0.864,respectively.Conclusion:According to our results,both screening tools are well correlated with the ESPEN criteria for the definition of malnutrition,while NRS2002 is easier to operate with shorter time,therefore,NRS2002 can identify the malnourished cancer patients more conveniently.
