摘　　要：目的分析特发性膜性肾病(IMN)中足细胞数目与ɑ3β1整合素及临床指标的关系。方法采集2016年2月—2017年3月中国医科大学附属盛京医院肾内科经临床和肾活检确诊的IMN住院患者30例的活检肾组织石蜡切片(病例组),同期于医院泌尿外科行肾脏肿瘤手术患者30例,采取远离肿瘤的正常区域肾组织(对照组)。用WT1标记足细胞核,用免疫组化法检测WT1、ɑ3β1整合素的表达量。比较2组足细胞数量、α3β1整合素表达水平,并分析足细胞数量与α3β1整合素的关系。结果病例组足细胞绝对数目、相对密度较对照组减少(t/P=8.467/0.000,2.650/0.012),且其绝对数目与相对密度分别与ɑ3β1整合素呈正相关(r/P=0.552/0.000、0.447/0.000),足细胞绝对数目、相对密度与IMN患者24 h尿蛋白呈负相关(r/P=-0.551/0.002、-0.512/0.004),而与肌酐、白蛋白无明显相关性(P>0.05)。结论特发性膜性肾病中足细胞绝对数目及相对密度均下降,且ɑ3β1整合素表达量也明显降低,随着足细胞数量的下降,24 h尿蛋白随之增多。Objective To analyze the relationship between the number of podocytes and the integrin and clinical indexes in idiopathic membranous nephropathy(IMN).Methods From February 2016 to March 2017,paraffin sections(case group)were collected from 30 in-patients with IMN confirmed by clinical and renal biopsy in Kidney Department of Shengjing Hospital Affiliated to China Medical University.At the same time,30 patients underwent renal tumor operation.The normal renal tissue far away from the tumor was taken(control group).WT1,α3β1 were detected by immunohistochemistry.The number of podocytes and the expression level ofα3β1 integrin were compared between the two groups,and the relationship between the number of podocytes andα3β1 integrin was analyzed.Results The absolute number and relative density of podocytes in the case group were lower than those in the control group(t/P=8.467/0.000,2.650/0.012),and their absolute number and relative density were positively correlated with the integrin(r/P=0.552/0.000,0.447/0.000).The absolute number and relative density of podocytes were negatively correlated with 24 h urinary protein(r/P=-0.551/0.002,-0.512/0.004),but not with creatinine and albumin(P>0.05).Conclusion In idiopathic membranous nephropathy,the absolute number and relative density of podocytes decreased,and the expression of integrin(α3β1)also decreased.With the decrease of podocyte number,24-h urine protein increased.
