CEUS引导下经皮微波消融治疗晚期肝脏恶性肿瘤的疗效评价

摘　　要：目的探究超声造影(CEUS)引导下经皮微波消融治疗晚期肝脏恶性肿瘤的临床疗效。方法选取本院2017年1月至2018年4月收治的晚期肝脏恶性肿瘤患者82例,根据随机序贯法分为对照组和观察组,各41例。对照组采用手术切除联合氩氦刀术中冷冻治疗,观察组采用CEUS引导下经皮微波消融治疗,比较分析两组近期疗效、术后并发症发生率、术后住院时间以及1年生存率。结果两组病情缓解率比较差异无统计学意义;观察组1年生存率高于对照组,术后并发症发生率低于对照组,住院时间短于对照组,比较差异均具有统计学意义(P<0.05)。结论对晚期肝脏恶性肿瘤患者实施CEUS引导下经皮微波消融术能够在保证临床疗效的同时,减少术后并发症,缩短患者术后住院时间,提高1年生存率,值得临床推广使用。Objective To investigate the clinical efficacy of percutaneous microwave ablation guided by contrast-enhanced ultrasound(CEUS)in the treatment of advanced hepatic malignancies.Methods 82 patients with advanced hepatic malignancies who were treated from January 2017 to April 2018 in our hospital were enrolled.They were divided into the control group and the observation group according to the randomized sequential method,with 41 cases in each group.The patients underwent cryoablation combined with argon-helium cryoablation.The observation group underwent CEUS-guided percutaneous microwave ablation.The short-term efficacy,postoperative complication rate,postoperative hospital stay and 1-year survival rate were compared between two groups.Results There was no significant difference in the remission rate between the two groups.The 1-year survival rate of the observation group was higher than that of the control group.The postoperative complication rate was lower than that of the control group,and the hospitalization time was shorter than the control group,and there was statistical significant difference(P<0.05).Conclusion CEUSguided percutaneous microwave ablation in patients with advanced hepatic malignancies can ensure the clinical efficacy,reduce postoperative complications,shorten postoperative hospital stay,and improve 1-year survival rate,which is worthy of clinical promotion.
