超声造影联合血清甲状腺球蛋白抗体检测在甲状腺癌诊断中的应用价值

摘　　要：目的探讨超声造影联合血清甲状腺球蛋白抗体(TgAb)检测在甲状腺癌诊断中的应用价值。方法收集90例甲状腺结节患者的临床资料,患者术前均接受过超声造影和血清TgAb检测。以术后病理诊断结果为金标准,比较超声造影、血清TgAb及二者联合检测诊断甲状腺癌的准确度。结果术后病理诊断结果显示,90例甲状腺结节患者中,甲状腺良性结节患者38例,甲状腺癌患者52例。超声造影检查、血清TgAb检测对甲状腺癌的诊断结果与术后病理诊断结果的一致性均较好(P﹤0.05);超声造影联合血清TgAb检测对甲状腺癌的诊断结果与术后病理诊断结果的一致性最佳(P﹤0.05)。超声造影联合血清TgAb对甲状腺癌的诊断价值优于任一方式单独诊断的结果(P﹤0.05)。结论超声造影联合血清TgAb检测可有效提高诊断甲状腺癌的准确度,对于临床甲状腺癌的诊治具有重要的参考意义。Objective To investigate the value of contrast-enhanced ultrasound(CEUS)combined with serum thyroglobulin antibody(TgAb)in the diagnosis of thyroid carcinoma.Method The clinical data of 90 patients with thyroid nodules were collected.All patients completed CEUS and serum TgAb test before operation.With postoperative pathology results as the gold standard,the accuracy of CEUS,serum TgAb as well as the combination of the two examinations for diagnosis of thyroid carcinoma were compared.Result Postoperative pathology findings indicated that there were 38 cases with benign nodules and 52 cases with malignant tumors among the 90 cases of thyroid nodules.Relatively high consistency between the findings identified by CEUS and serum TgAb and postoperative pathology results was noted(P<0.05).Besides,the optimal consistency was observed for CEUS combined with serum TgAb relative to postoperative pathology findings in the diagnosis of thyroid carcinoma(P<0.05).The diagnostic value of CEUS combined with serum TgAb in detecting thyroid carcinoma was higher than that of either method used alone(P<0.05).Conclusion CEUS combined with serum TgAb can effectively improve the clinical diagnostic accuracy for thyroid carcinoma,which is of great significance for clinical diagnosis and treatment of thyroid carcinoma.
