摘 要:目的探讨计算机断层扫描(CT)动态增强扫描检查在早期肺癌诊断中的应用价值。方法选取100例孤立性肺结节患者,以病理诊断结果为金标准,分析CT动态增强扫描检查对早期肺癌的诊断价值。结果100例孤立性肺结节患者中,经病理学检查确诊,早期肺癌(肺腺癌)患者80例,肺部良性病变患者20例。CT动态增强扫描检查诊断早期肺癌的灵敏度、特异度、准确度、阳性预测值和阴性预测值分别为93.75%、85.00%、92.00%、96.15%和77.27%。早期肺癌的血容量明显少于肺部良性病变,平均通过时间明显短于肺部良性病变(P﹤0.01);早期肺癌的血流量和表面渗透性均高于肺部良性病变(P﹤0.05)。血容量、血流量、表面渗透性和平均通过时间诊断早期肺癌的受试者工作特征(ROC)曲线下面积分别为0.737、0.724、0.779和0.946。结论 CT动态增强扫描检查在早期肺癌的诊断中有较好的应用价值,值得在临床中推广应用。Objective To explore the value of dynamic contrast-enhanced computed tomography(CT)in the diagnosis of early lung cancer.Method This analysis included 100 patients with solitary pulmonary nodules,which were examined with dynamic contrast-enhanced CT with pathology results as the gold standard,to determine the diagnostic value of the enhanced scan for early lung cancer.Result Through pathological examination,80 cases were identified among the 100 patients as early lung cancer(lung adenocarcinoma)and 20 cases were with benign lung lesions.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of dynamic contrast-enhanced CT in the diagnosis of malignant lesions were 93.75%,85.00%,92.00%,96.15%and 77.27%,respectively.The blood volume and mean transit time(MTT)in early lung cancer patients were evidently less or shorter compared to that in those with benign lung lesions(P<0.01),while the blood flow and surface permeability were significantly higher in early lung cancer than in benign lung lesions(P<0.05).The areas under receiver operating characteristic(ROC)curves of diagnosis of malignant lesions by blood volume,blood flow,surface permeability and MTT were 0.737,0.724,0.779 and 0.946,respectively.Conclusion Dynamic contrast-enhanced CT scan demonstrates fairly good diagnostic value in predicting early lung cancer.
