翼状胬肉手术自体结膜移植与生物羊膜移植的效果比较

摘　　要：目的:比较翼状胬肉切除联合自体结膜移植术与联合生物羊膜移植术的临床疗效。方法:前瞻性病例对照研究。收集郑州人民医院2017年1月至2018年1月原发性翼状胬肉58例(61眼)随机分为两组:自体结膜移植组30例(31眼)和生物羊膜移植组28例(30眼)。术后随访1年,比较两组的复发率及角膜上皮愈合时间。结果:自体结膜移植组复发率为6.5%(2/31),低于生物羊膜移值组的复发率17.7%(5/30),两组复发性相比差异有统计学意义(χ^2=4.586,P=0.012)。自体结膜移植组角膜上皮愈合时间平均(3.12±1.05)天,短于生物羊膜移植组的(4.67±0.98)天,两组相比差异有统计学意义(t=6.785,P=0.003)。结论:翼状胬肉切除术联合自体结膜移植术治疗翼状胬肉复发率更低,术后角膜上皮愈合更快。Objective To compare the clinical efficacy between autologous conjunctiva transplantation and biological amniotic membrane transplantation for pterygium.Methods A prospective case-control study.Total of 61 eyes of 58 patients with primary pterygium from Jan.2017 to Jan.2018 in the People’s Hospital of Zhengzhou were collected.All patients were randomly divided into two groups.The autologous conjunctiva transplantation group included 31 eyes of 30 patients and the biological amniotic membrane transplantation group included 30 eyes of 28 patients.All patients were followed up for 1 year.The recurrence rate and the time of corneal epithelial healing were compared.Results The recurrence rate was 6.5%(2/31)in autologous conjunctiva transplantation group which was lower than 17.7%(5/30)in amniotic membrane transplantation group,the difference was statitically significant(χ^2=4.586,P=0.012).The average time of corneal epithelial healing was(3.12±1.05)days in autologous conjunctiva transplantation group,which was shorter than(4.67±0.98)days in amniotic membrane transplantation group,the difference was statitically significant(t=6.785,P=0.003).Conclusion There is a lower recurrence rate and shorter corneal epithelial healing time in pterygium resection combined with autologous conjunctiva transplantation.
