摘　　要：目的探讨前列腺血管内NK/T细胞淋巴瘤的临床病理及免疫组织化学特征。方法收集并分析武汉大学中南医院病理科诊断的1例前列腺血管内NK/T细胞淋巴瘤患者的临床病理资料,并结合文献进行复习总结。结果患者,男,88岁,因"排尿困难1月余,血尿1周"入院,泌尿系B超提示前列腺增生症,遂行经尿道前列腺等离子电切术。术后病检示前列腺间质血管腔内见异型淋巴细胞;免疫组化示CD3、CD8、CD30、CD56、TIA-1、granzyme B阳性,CK、CD20、CD4、CD7阴性。原位杂交示EBER阳性。诊断为前列腺血管内NK/T细胞淋巴瘤。术后患者拒绝接受治疗,5个月后因肿瘤累及多个脏器导致多器官功能衰竭死亡。结论血管内NK/T细胞淋巴瘤是一种十分罕见的血管内淋巴瘤,诊断时需结合临床、病理及实验室检查综合考虑,该肿瘤具有较强的侵袭性,预后较差,治疗多采用以CHOP方案为主的化疗。Objective To report the first case of intravascular NK/T cell lymphoma of prostate,and review the related literature to investigate the clinicopathological and immunohistochemical features of this tumor.Methods The clinical manifestations,pathological changes and immunohistochemical features of the intravascular NK/T cell lymphoma of prostate that diagnosed by department of pathology,Zhongnan Hospital of Wuhan University were collected and analyzed.Results A 88-year-old male presented with dysuria for a month,and hematuria for a week.Urinary system type-B ultrasonic revealed benign prostatic hyperplasia.Thereafter,transurethral plasma prostatectomy was performed.Histopathology showed that atypical lymphocytes were found in blood vessel of prostate stroma.Immunohistoch-emical results showed that CD3,CD8,CD30,CD56,TIA-1,granzyme B were positive,while CK,CD20,CD4,CD7 were negative.Hybridization in situ showed that EBER was positive.The final diagnosis was intravascular NK/T cell lymphoma of prostate.The patient refused to receive treatment after operation,so five months later,the patient died of multiple organ failure due to tumor involvement of multiple organs.Conclusions Intravascular NK/T cell lymphoma is a very rare intravascular lymphoma,which needs to be considered in combination with clinical,pathological and laboratory results.The tumor is aggressive and has a poor prognosis.It is mainly treated by CHOP chemotherapy.
