循经远取动法结合常规针刺治疗原发性三叉神经痛临床观察

摘　　要：目的观察循经远取动法结合常规针刺治疗原发性三叉神经痛的临床疗效。方法将90例原发性三叉神经痛患者随机分为常规组、循经远取组、循经远取动法组,每组30例。常规组按照第10版《针灸学》选穴及操作治疗,循经远取组在常规组基础上循经远取治疗,循经远取动法组在循经远取组基础上配合动法治疗;3组疗程周期均为4周,采用可视化视觉模拟尺度评分(Visual Analogue Scales,VAS)及简化马克吉尔问卷(The Short-form McGill Pain Questionnaire,SF-MPQ)对结果进行评估。结果常规组总有效率为88.8%,循经远取组为92.6%,循经远取动法组为96.6%,3组比较差异均具有统计学意义(P<0.05)。与治疗前VAS评分及简化马克吉尔量表评分相比,3组患者首次治疗后即刻、治疗结束后的评分均降低,组内比较差异具有统计学意义(P<0.05);循经远取动法组疗效显著于循经远取组及常规组,组间比较差异具有统计学意义(P<0.05)。治疗结束后6个月进行随访时,常规组及循经远取组各项指标均有不同程度的回升,组内比较差异具有统计学意义(P<0.05),循经远取动法组与治疗后相比差异无统计学意义(P>0.05)。结论循经远取动法结合常规针刺能够明显减轻原发性三叉神经痛患者的疼痛,即刻及远期治疗效果显著。Objective To observe the clinical efficacy of needling distal point along meridian plus exercising combined acupuncture by routine way in treatment of primary trigeminal neuralgia.Methods Ninety cases of primary trigeminal neuralgia patients were divided into routine group,meridianfollowing group and meridian-following plus exercising group.Each group has 30 cases.The routine group was treated by routine way.The meridian-following group was treated by needling distal point along meridians with Liangqiu or Wenliu combined routine acupuncture.Meridian-following plus exercising group was treated on the basement of the meridian-following group plus exercising at the meantime.Every group had 4 weeks as a course of treatment and the final efficacy was judged by visual analogue scales(VAS)and short-form McGill Pain questionnaire(SF-MPQ).Results The total effective rate was 88.8%in routine group,92.6%in meridian-following group,and 96.6%in the meridian-following plus exercising group,the difference between the three groups was statistically significant(P<0.05).Compared with the VAS score and SF-MPQ scores before treatment,the scores of the three groups decreased immediately after the first treatment and after the treatment,and there was a significant difference between the three groups(P<0.05).The curative effect of the long-distance removal along meridians group was significantly better than that of the meridian-following plus exercising group and there was a significant difference between the three groups(P<0.05).After 6 months of follow-up,the indexes of the routine group and the meridianfollowing group increased in varying degrees.There was significant difference between the two groups(P<0.05).There was no significant difference between the meridian-following plus exercising group(P>0.05).Conclusion Needling distal point along meridian plus exercising combined acupuncture by routine way can obviously relieve the pain of patients with primary trigeminal neuralgia,and the immediate and long-term therapeutic effect is r
