八珍汤加减对原发性肝癌TACE术后不良反应的临床观察

摘　　要：目的评价八珍汤加减对原发性肝癌TACE术后不良反应的影响。方法选取2018年1月~2019年1月我院住院治疗的原发性肝癌TACE术后患者60例,按1:1随机数字表法分为治疗组和对照组。两组均给予TACE治疗,治疗组在常规治疗基础上服用八珍汤加减治疗,比较两组术前3 d和术后28 d临床不良反应(疼痛、恶心呕吐、肝功能毒性、发热)的发生情况,比较两组患者术前3 d和术后28 d WBC、RBC、ALT、AST变化情况及对患者生活质量的影响。结果治疗后两组疼痛、恶心呕吐、谷丙转氨酶上升、发热比较,差异有统计学意义(P<0.05);与治疗前比较,治疗后WBC、RBC、ALT、AST均见明显好转(P<0.05),但治疗组好转优于对照组(P<0.05);治疗组八珍汤加减应用后KPS评分明显高于对照组(P<0.05)。结论八珍汤加减改善原发性肝癌TACE术后不良反应疗效确切,提高了患者的生活质量,具有良好的临床应用价值。Objective To evaluate the effect of modified Bazhen decoction on adverse reactions after transcather arterial chemoembolization(TACE)in primary liver cancer(PLC).Methods 60 patients with PLC after TACE hospitalized in our hospital from January 2018 to June 2019 were selected and divided into a treatment group and a control group using the random number table method,with 30 cases in each group.Both groups were treated with TACE,and the treatment group received modified Bazhen decoction on the basis of conventional treatment.The clinical adverse reactions(pain,nausea and vomiting,liver toxicity,and fever)were compared between the two groups 3 days before surgery and 28 days after surgery.The changes of white blood cell(WBC),red blood cell(RBC),alanine aminotransferase(ALT),and aspartate aminotransferase(AST)and their effects on quality of life were compared between the two groups 3 days before and 28 days after surgery.Results After treatment,there were significant differences in pain,nausea and vomiting,increase of alanine amin otransferase,and fever between the two groups(P<0.05);compared with before treatment,WBC,RBC,ALT,and AST showed significant improvement after treatment(P<0.05),but the improvement in the treatment group was better than that in the control group(P<0.05);the Karnofsky performance status(KPS)score of modified Bazhen decoction in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion Modified Bazhen decoction can improve the adverse effects of PLC after TACE,and promote the quality of life of patients.It has good clinical application value.
