滤过泡针刺分离联合球结膜下5-FU注射治疗青光眼小梁切除术后早期滤过泡功能不良的效果

摘　　要：目的 探讨滤过泡针刺分离联合球结膜下5-FU注射治疗青光眼小梁切除术后早期滤过泡功能不良的效果.方法 将我院眼科2016年1月至2019年1月收治的84例(84眼)青光眼行小梁切除术后早期滤过泡功能不良患者随机分为对照组和观察组,各42例(42眼).对照组采用传统眼球按摩及滤过泡针刺分离治疗,观察组在对照组基础上采用球结膜下5-FU注射治疗.比较两组的治疗效果.结果 治疗后6个月,观察组形成功能性滤过泡的成功率高于对照组(P<0.05).观察组治疗后1、7 d及1、3、6个月的眼压均低于对照组(P<0.05).观察组治疗后1、3、6个月的视力分布情况优于对照组(P<0.05).两组并发症发生率无显著差异(P>0.05).结论 滤过泡针刺分离联合球结膜下5-FU注射治疗青光眼小梁切除术后早期滤过泡功能不良的效果良好,可改善术后眼压、视力情况,安全性高.Objective To investigate the effect of acupuncture separation of filtering bleb combined with 5-FU injection under bulbar conjunctiva in the treatment of glaucoma patients with early filtering bleb dysfunction after trabeculectomy.Methods From January 2016 to January 2019,84 glaucoma patients(84 eyes)with early filtering bleb dysfunction after trabeculectomy enrolled in ophthalmology department of our hospital were randomly divided into control group and observation group,with 42 cases(42 eyes)in each group.The control group was treated with traditional eyeball massage and acupuncture separation of filtering bleb,while the observation group was treated with 5-FU injection under bulbar conjunctiva on the basis of the control group.The therapeutic effects of the two groups were compared.Results Six months after treatment,the success rate of forming functional filtering blebs in the observation group was higher than that in the control group(P<0.05).The intraocular pressure at 1,7 days and 1,3,6 months after treatment of the observation group were lower than those of the control group(P<0.05).At 1,3,6 months after treatment,the visual acuity distribution of the observation group were better than those of the control group(P<0.05).There were no significant differences in the incidences of complications between the two groups(P>0.05).Conclusion Acupuncture separation of filtering bleb combined with 5-FU injection under bulbar conjunctiva in the treatment of glaucoma patients with early filtering bleb dysfunction after trabeculectomy has a good effect,can improve intraocular pressure and visual acuity,with high safety.
