摘 要:目的探讨依诺肝素钠-聚甲基丙烯酸甲酯(ES-PMMA)骨水泥在动物体内释放特性和术后持续释放依诺肝素钠药物浓度。方法将20只家兔[望都县彤辉养殖,动物合格证编号为191106,体重(2.5±0.5)kg,兔龄3~6个月]随机分为A、B、C、D 4组,A组为对照组,B、C、D组为实验组,制作标准ES-PMMA骨水泥圆柱体试件,实验组每40 g骨水泥加入依诺肝素钠(ES)的量分别为4000、8000、12000 AXaIU,对照组不添加依诺肝素钠。手术方式将试件分别置于新西兰雄兔右侧股骨髓腔内,以不可吸收缝线捆绑,内置负压引流管收取引流液,实验组连续10 d测定引流液中的依诺肝素钠药物浓度。统计学方法采用单因素方差分析。结果所有兔均存活饲养至实验结束,ES加入为4000、8000、12000 AXaIU的实验组24 h后释放量分别为0.713、1.157、1.662 IU/ml,各实验组均在体内放置24 h后达到ES释放浓度的最高峰,此后浓度释放逐渐衰减,在第4~10天释放趋于稳定。结论ES-PMMA骨水泥试件植入新西兰兔体内后能够从局部释放出来,发挥局部抗凝作用。Objective To study the release characteristics of sodium enoxaparin polymethyl methacrylate(es-pmma)bone cement in animals and the sustained release concentration of enoxaparin sodium after operation.Methods Twenty rabbits were randomly divided into four groups:group A,control group;groups B,C and D:experimental groups.The standard es-pmma bone cement cylinder specimen was made.The amount of enoxaparin sodium(ES)added to every 40 g bone cement in the experimental groups was 4000,8000 and 12000 AXaIU respectively,and the control group was not given enoxaparin sodium.The test pieces were placed in the right femoral marrow cavity of New Zealand male rabbits,bound with non absorbable suture,and the drainage fluid was collected by a built-in negative pressure drainage tube.The concentration of enoxaparin sodium in the drainage fluid was measured in the experimental groups for 10 consecutive days.Results All the animals survived until the end of the experiment.The release of ES in the experimental groups with 4000,8000 and 12000 AXaIU in 24 h was 0.713,1.157 and 1.662 IU/ml,respectively.The release of ES in each experimental group reached the peak at 24 h in vivo.Thereafter,the release of ES gradually declined,and the release of ES in the 4th-10th days tended to be stable.Conclusion After implanted into New Zealand rabbits,es-pmma bone cement can be released from the local area and may play a local anticoagulant role.
