针灸治疗消化系统适宜病症的现代文献研究

摘　　要：目的:回顾消化系统适宜病症针灸治疗方案的现有证据并整合最佳证据,对临床穴位进行研究。方法:选取慢性胆囊炎、胆结石、慢性胃炎、胃下垂、肠梗阻、便秘的临床文献,运用临床流行病学和循证医学方法对文献进行分析,统计整理符合纳入标准的文献特征,初步研究针灸治疗适宜病症的取穴规律。结果:针灸上述病症的治疗中存在诸多问题,在经脉主治方面无明显规律可循,上述病症中有88%的高频穴位都与疾病所在系统处于相同或相近的神经节段区,而少数处于较远神经节段区的穴位仅占12%。结论:腧穴的主治范围主要由相关神经节段的支配空间决定,针刺的节段性效应及整体性效应可作为针灸取穴理论的有力补充。Objective:To review the existing evidence of acupuncture treatment for digestive system indications,and integrate the best evidence to study the clinical acupoint.Methods:Selecting the clinical literature of chronic cholecystitis,gallstone,chronic gastritis,gastroptosis,intestinal obstruction and constipation,using clinical epidemiology and evidence-based medicine to analyze the literature,statistically collation of literature characteristics in line with the inclusion criteria,preliminary studying the regularity of acupoint selection for indications.Results:There are many problems in the treatment of acupuncture indications mentioned above,and also there is no obvious rule to follow in the main treatment of meridians.A total of 88%of the high-frequency acupoints in the acupuncture treatment of above-mentioned indications are in the same or similar ganglion region as the disease system,while a few acupoints in the farther nerve segment only account for 12%.Conclusion:The main scope of acupoints is mainly determined by the dominating space of the relevant nerve segments.The segmental effects and overall effects of acupuncture can be used as a powerful supplement to the theory of acupoint selection.
