肝癌患者血清miR-122-5p miR-205-5p miR-486-5p的表达及临床意义

摘　　要：目的探讨肝癌患者血清miR-122-5p、miR-205-5p和miR-486-5p的表达及其临床意义.方法对51例肝癌患者(肝癌组)、51例慢性肝病患者(肝病组)以及51名健康体检者(对照组)采用实时荧光定量聚合酶链式反应检测miR-122-5p、miR-205-5p和miR-486-5p表达量,并分析其与临床病理参数的关系及对肝癌诊断的评估价值.结果肝癌组miR-122-5p、miR-205-5p、miR-486-5p表达显著低于肝病组和对照组(P<0.05),肝病组显著低于对照组(P<0.05).肝癌组血清miR-122-5p、miR-205-5p、miR-486-5p的表达在临床肿瘤大小、肿瘤分期以及分化程度方面存在明显差异(P<0.05),在性别、年龄、病理类型方面无明显差异(P>0.05),临床肿瘤大小≤4 cm、肿瘤分期为Ⅰ/Ⅱ期、分化程度低的肝癌患者血清miR-122-5p、miR-205-5p、miR-486-5p的表达显著高于肿瘤大小>4 cm、肿瘤分期为Ⅲ/Ⅳ期、分化程度为中高的患者(P<0.05).受试者工作特征曲线分析结果显示,血清miR-122-5p、miR-205-5p、miR-486-5p及三者联合对肝癌诊断的曲线下面积分别为0.952、0.779、0.734、0.975;各指标评估肝癌的价值为:三者联合>miR-122-5p>miR-205-5p>miR-486-5p.结论肝癌患者血清miR-122-5p、miR-205-5p和miR-486-5p的表达较高,且与肝癌肿瘤大小、分期、分化程度等病理特征关系密切,对临床肝癌早期诊断具有指导意义.Objective To investigate the expression of serum miR-122-5p,miR-205-5p and miR-486-5p in patients with HCC and its clinical significance.Methods Stratified random selection of 51 HCC patients(liver cancer group),51 patients with chronic liver disease(liver disease group),and 51 healthy people(control group).Real-time quantitative PCR was used to compare the expression levels of miR-122-5p,miR-205-5p and miR-486-5p in each group,and to analyze its relationship with clinicopathological parameters and evaluation of HCC value.Results The expression levels of miR-122-5p,miR-205-5p,and miR-486-5p in the liver cancer group were significantly lower than those in the liver disease group and the control group(P<0.05),and the liver disease group was significantly lower than the control group(P<0.05).The expressions of serum miR-122-5p,miR-205-5p,miR-486-5p in the liver cancer group were significantly different in clinical tumor size,tumor stage,and degree of differentiation(P<0.05),and in terms of gender,age,and pathological type had no significant difference(P>0.05).The expression of miR-122-5p,miR-205-5p,miR-486-5p in serum of patients with liver cancer with clinical tumor size≤4 cm,tumor stage Ⅰ/Ⅱ,and low degree of differentiation is significantly higher than patients with tumor size>4 cm,tumor stageⅢ/Ⅳ,and moderately high differentiation(P<0.05).The results of ROC curve analysis showed that the serum AUR-122-5p,miR-205-5p,miR-486-5p and the three combined AUC were three>miR-122-5p>miR-205-5p>miR-486-5p.Conclusions The expression of miR-122-5p,miR-205-5p and miR-486-5p in sera of patients with liver cancer is high,which are closely related to pathological features such as tumor size,TNM stage and tumor differentiation of liver cancer,and have guiding significance for early diagnosis of clinical liver cancer.
