摘 要:目的探究唇腭裂术后瘢痕对上颌骨生长影响的作用机制。方法在原腭裂上颌骨有限元模型上,通过ANSYS软件中布尔操作技术进行上唇软组织的添加,形成含上唇软组织的唇腭裂上颌骨三维有限元模型,在模型上同时添加上唇压力及腭部瘢痕力,观察上颌骨的变化情况。结果两侧上颌骨均出现形变,由前向后形变幅度逐渐降低,且存在不对称性。三维方向上形变比较:X轴>Z轴>Y轴。结论唇腭裂手术瘢痕对上颌骨生长产生明显抑制,由前向后抑制程度逐渐降低,牙槽突前段受抑制最明显;上颌骨在三维方向上生长呈现出不对称性;三维方向抑制程度比较:横向>矢状向>垂直向。Objective To explore the mechanism of scar on maxillary growth after cleft lip and palate surgery.Methods On the original finite element model of maxilla with cleft palate,soft tissue of upper lip was added by Boolean operations in ANSYS to form a 3D finite element model of maxilla with soft tissue of upper lip.Upper lip pressure and palatal scar force were applied to the model at the same time,and observing maxillary deformation.Results Deformation and asymmetric growth occurred in bilateral maxilla,and deformation trend was gradually decreased from anterior to posterior.Comparison of deformations in three-dimensional direction:X axis>Z axis>Y axis.Conclusion Surgical scar of cleft lip and palate significantly inhibites maxillary growth.Inhibition extent is gradually decreased from anterior to posterior,and anterior segment of alveolar process is the most inhibited.Maxillary growth showes asymmetry in three-dimensional direction.Comparison of inhibition extent in three-dimensional direction:transverse>sagittal>vertical.
