摘 要:目的探讨放散状冲击波联合经皮神经电刺激治疗颞下颌关节紊乱病的疗效。方法选取TMDⅠ类患者60例,随机法分为经皮神经电刺激组、放散状冲击波组和联合治疗组。经皮神经电刺激组给予经皮神经电刺激治疗+手法治疗,放散组给予放散状冲击波治疗+手法治疗,联合治疗组给予经皮神经电刺激治疗+放散状冲击波治疗+手法治疗。在治疗前、治疗结束时记录所有患者的Fricton颞下颌关节紊乱指数、VAS评分及开口度,在治疗过程中记录患者的不良反应发生情况。结果联合治疗组PI、CMI和VAS评分从治疗前至治疗后有较大幅度降低,3组治疗后PI、CMI和VAS评分比较差异有统计学意义(P<0.05);3组PI从治疗前至治疗后呈现较小幅度降低,3组治疗后比较,差异均无统计学意义(P>0.05)。治疗后,3组患者的开口度均较治疗前改善,治疗后3组开口度比较,差异有统计学意义(P<0.05)。结论放散状冲击波联合经皮神经电刺激治疗能够早期缓解疼痛,改善关节功能。Objective To evaluate the effect of radial extracorporeal shock waves combined with transcutaneous electrical nerve stimulation in the treatment of temporomandibular disorders.Methods Sixty patients were divided into transcutaneous electrical nerve stimulation group(TENS)、radial extracorporeal shock wave treatment(REWST)group and combination treatment(CT)group.TENS group accepted transcutaneous electrical nerve stimulation and physiotherapy(soft tissue massage),RESWT group accepted physiotherapy and radial extracorporeal shock wave treatment,CT group accepted TENS,RESWT and physiotherapy treatment.Fricton’s index and Opening rang(OR)was used to evaluate TMJ function.Evaluated all subjects’pain levels with visual analogue scales(VAS).During the treatment,the occurrence of adverse reactions was recorded.Results After treatment the three groups had statistically significant(P<0.05),however,DI among the three groups had no statistically significant difference(P>0.05).After treatment,OR among the three groups had statistically significant(P<0.05).There were no significant difference in adverse reaction rates among the three groups(P>0.05).Conclusion Radial extracorporeal shock waves combined with transcutaneous electrical nerve stimulation can relieve pain in the early stage and improve joint function.
