摘 要:目的探讨血清基质金属蛋白酶-2(MMP-2)、鳞状细胞癌抗原(SCCA)表达水平在肺癌诊断及病情评估中的价值。方法以我院2018年2月至2019年5月收治的123例肺癌患者为肺癌组,以同期在我院治疗的肺炎患者130例为肺炎组。检测2组血清MMP-2、SCCA表达水平,比较分析2组及不同TNM分期患者血清MMP-2、SCCA表达水平,评估血清MMP-2、SCCA在肺癌诊断及病情评估中的价值。结果肺癌组血清MMP-2(4.54±1.21)μg/L、SCCA(182.34±15.27)μg/L)表达水平均明显高于肺炎组(1.17±0.63)μg/L、(127.65±11.23)μg/L,差异有统计学意义(P均<0.05)。血清MMP-2、SCCA联合检测阳性率(85.4%)明显高于MMP-2(69.9%)、SCCA(56.9%)单一指标,差异均有统计学意义(P均<0.05)。血清MMP-2、SCCA联合检测敏感性(85.4%)、特异性(94.6%)、准确率(88.4%)、阳性预测值(93.8%)、阴性预测值(87.2%)明显高于单一检测指标MMP-2(69.9%、72.3%、71.1%、70.5%、71.8%)、SCCA(56.9%、83.1%、70.4%、76.1%、67.1%)(P均<0.05)。肺癌Ⅰ、Ⅱ、Ⅲ、Ⅳ期血清MMP-2、SCCA表达水平依次为(2.58±0.72)和(161.36±13.63)μg/L、(4.02±1.13)和(198.57±15.31)μg/L、(5.06±1.32)和(223.62±17.14)μg/L、(8.03±2.12)和(242.17±23.81)μg/L,两两比较差异均有统计学意义,且随着期别的进展呈逐级递增(P均<0.05)。结论血清MMP-2、SCCA联合检测在肺癌早期诊断及TNM分期中具有重要价值。Objective To investigate the value of serum matrix metalloproteinase-2(MMP-2)and squamous-cell carcinoma antigen(SCCA)expression levels in the diagnosis and prognosis evaluation of lung cancer.Methods The 123 cases of lung cancer patients in our hospital from February 2018 to May 2019 were divided into the lung cancer group,and 130 cases of pneumonia patients treated in our hospital during the same period were divided into the pneumonia group.The serum MMP-2 and SCCA expression levels of the two groups were detected,serum MMP-2 and SCCA expression levels in the two groups and patients with different TNM stages,and the value of serum MMP-2 and SCCA in the diagnosis and condition evaluation of lung cancer were compared and analyzed.Results The expression levels of serum MMP-2[(4.54±1.21)μg/L]and SCCA[(182.34±15.27)μg/L)]in the lung cancer group were significantly higher than those in the pneumonia group[(1.17±0.63)μg/L,(127.65±11.23)μg/L],the difference was statistically significant(P<0.05).The positive rate of combined detection of serum MMP-2 and SCCA(85.4%)was significantly higher than the positive rate of single index detection of MMP-2(69.9%)and SCCA(56.9%),the difference was statistically significant(P<0.05).The sensitivity(85.4%),specificity(94.6%),accuracy(88.4%),positive predictive value(93.8%),negative predictive value(87.2%)of serum MMP-2 and SCCA combined detection were significantly higher than the single detection index serum MMP-2(69.9%,72.3%,71.1%,70.5%,71.8%),SCCA(56.9%,83.1%,70.4%,76.1%,67.1%)(P<0.05).The expression levels of serum MMP-2 and SCCA in theⅠ,Ⅱ,Ⅲ,Ⅳstage lung cancer were respectively(2.58±0.72)and(161.36±13.63)μg/L,(4.02±1.13)and(198.57±15.31)μg/L,(5.06±1.32)and(223.62±17.14)μg/L,(8.03±2.12)and(242.17±23.81)μg/L,the difference among all the different stages were obvious(P<0.05).Conclusion The combined detection of serum MMP-2 and SCCA is of great value in the early diagnosis of lung cancer and TNM staging.
