计算机断层扫描引导下经皮肺穿刺活检术的临床应用现状及病理结果浅析

摘　　要：目的总结计算机断层扫描(CT)引导下经皮肺穿刺活检术的临床应用现状,分析其病理结果及并发症发生情况。方法回顾性分析1268例行CT引导下经皮肺穿刺活检术患者的临床病理资料,并抽取部分医院同期开展CT引导下经皮肺穿刺活检术的情况进行分析。结果本组1268例患者的穿刺成功率100.0%,一次穿刺出现气胸87例(6.9%)、出血315例(24.8%)、胸膜反应9例(0.7%)。全组1268例患者结果回报1251例,穿刺诊断率98.7%,其中恶性肿瘤835例(66.7%)、良性疾病416(33.3%),恶性肿瘤中腺癌和鳞癌为主。市、县级医院CT引导下经皮肺穿刺活检术开展较少。结论肺部占位性疾病中恶性肿瘤比例较高,应尽早明确病理诊断。CT引导下经皮肺穿刺活检术穿刺成功率高,并发症少,对肺部占位性疾病的诊治价值突出,但部分医院开展情况较差。Objective To summarize the clinical application of computed tomography(CT)guided percutaneous lung biopsy and to analyze the pathological results and complications.Methods The clinical and pathological data of 1268 patients under CT guided percutaneous lung biopsy were analyzed,and the CT guided percutaneous lung biopsy in the other hospitals were analyzed too.Results In this group,the puncture success rate was 100.0%,87 cases(6.9%)had pneumothorax,315 cases(24.8%)had bleeding,9 cases(0.7%)had pleura reaction.Of the 1268 cases,1251 cases had the pathological results,the diagnostic rate was 98.7%,including 835 cases(66.7%)of malignant tumor and 416 cases(33.3%)of benign disease.most city and county hospitals did not carry out this operation.Conclusion Malignant diseases account for a high proportion in lung space occupying diseases,and pathological diagnosis should be made as early as possible;CT guided percutaneous lung biopsy has a high success rate and small complications,which is of great value in the diagnosis and treatment,the development of some hospitals is poor.
