摘 要:目的:切入"肠-肾"尿酸(UA)排泄,结合体内外实验,从氧化应激介导的组织炎症损伤,观察菊苣促UA排泄的降UA药效。方法:10%高果糖饮水建立高尿酸血症(HUA)大鼠模型,生化检测血清尿酸(SUA)、尿尿酸(UUA)、粪便尿酸含量(FUA),计算尿尿酸清除率(CUUA),检测肠、肾组织中丙二醛(MDA)、超氧化物歧化酶(SOD)水平;Elisa检测血清D-乳酸、β微球蛋白及肠、肾组织中肿瘤坏死因子α(TNF-α)、白介素6(IL-6)水平。600μmol/L UA刺激Caco2、HKC细胞模拟高尿酸状态,以H2O2、N-乙酰-L-半胱氨酸为工具药,观察各处理组细胞中MDA、IL-6水平。结果:与模型组比较,各中药组可显著降低大鼠血清中D-乳酸、β微球蛋白及肠、肾组织MDA、TNF-α、IL-6表达(P<0.05,P<0.01),实验第20、30天,各中药组SUA水平显著降低,FUA水平显著升高(P<0.01,P<0.05);与HUA组比较,PUA+菊苣组可显著降低高尿酸刺激后Caco2、HKC细胞中MDA、IL-6水平(P<0.05)。结论:PUA状态下"肠-肾"UA排泄异常与氧化应激介导的"肠-肾"组织炎症损伤有关,菊苣通过改善"肠-肾"组织氧化应激性炎症损伤促UA排泄。Objective: To observe oxidative stress and inflammatory injury in the ‘intestine and kidney’ under the condition of hyperuricemia to explore the promotion of urate excretion intervened by Cichorii Herba Cichorii Radix combined with in vivo and in vitro experiments. Methods: Hyperuricemia rats were induced by 10% fructose drinking. The levels of serum uric acid(SUA), urinary uric acid(UUA) and fecal uric acid(FUA) were detected. The clearance rate of urine urate(CUUA) was calculated. The levels of malondialdehyde(MDA), superoxide dismutase(SOD), tumor necrosis factor α(TNF-α) and interleukin 6(IL-6) in the intestine and kidney were measured. The D-lactic acid level and the β microglobulin level in the serum were also detected by Elisa. The levels of MDA and IL-6 in Caco2 and HKC stimulated by 600μmol/L UA, H2O2 or N-acety-L-cysteine were detected, respectively. Results: Compared with model group, Cichorii Herba Cichorii Radix could significantly decrease the levels of serum D-lactic acid, serum β microglobulin and the expression of MDA, TNF-α, IL-6 in intestine and kidney(P<0.05, P<0.01), at the 20 th, 30 th day, Cihorii Herba Cichrii Radix could decreased the level of SUA, while increased the level of FUA(P<0.01, P<0.05). Compared with HUA group, PUA+Cichorii Herba Cichorii Radix could significantly decreased the levels of MDA, IL-6 in Caco2, HKC cells stimulated by the high level of UA. Conclusion: Decreased urate excretion is related to the inflammatory injury of the intestine and kidney induced by oxidative stress in hyperuricemia. The uricosuric effect of Cichorii Herba Cichorii Radix may be associated with inhibition of inflammatory injury by suppressing oxidative stress in the ’intestine and kidney’.
