摘　　要：慢性病监测管理信息系统是集病例报告、随访管理、数据管理和统计分析于一体的信息化系统,与全民健康信息平台共享数据资源。临床专家和疾控专家利用该系统为患者提供干预指导方案,社区医生提供随访干预服务,干预信息和效果同步至患者个人健康档案。该系统以患者健康管理数据为基础,结合统计图表和地理信息系统(geographic information system,GIS)等手段,可直观展示慢性病在人群、时间和地区的分布状态,为慢性病防控提供技术支撑。The chronic disease surveillance management information system described in this article is an information system integrating case report,follow-up management,data management and statistical analysis.It shares data resources with the National Health Information Platform.By using this system,clinical experts and CDC experts provide intervention guidance for patients,community doctors provide follow-up intervention services,the intervention information and effects are synchronized to the patient's personal health records.Based on patient health management data and combined with statistical charts and GIS,this system can intuitively display the distribution of chronic diseases among population,time and regions,and provide technical support for the prevention and control of chronic diseases.
