两种排龈法在口腔瓷冠修复中的应用效果比较

摘　　要：目的:探讨双线排龈法与Nd∶YAG激光排龈法在口腔瓷固定修复中的应用效果。方法:选择120例(179颗牙)前牙或前磨牙烤瓷冠修复病例,随机分为对照组(n=30例,92颗)和观察组(n=30例,87颗)。对照组采用双线排龈法治疗,观察组采用Nd∶YAG激光排龈法治疗,观察两组患者排龈时间,对比两组患者排龈前、排龈1周、1个月及3个月后的牙周组织健康状况,调查两组排龈后满意度。结果:观察组的排龈时间显著短于对照组(P<0.05);观察组排龈后疼痛等级改善效果显著优于对照组(P<0.05);排龈1周~3个月后,两组患者的探诊深度(PD)值呈先降低后提高趋势,组内不同时间点间对比,差异有统计学意义(P<0.05),观察组排龈1周、1个月后的PD值显著高于对照组(P<0.05);排龈1周、1个月及3个月后,两组患者的探诊出血(BOP)分级和牙龈指数(GI)分级结果对比,差异均无统计学意义(P>0.05);排龈后,观察组的牙龈止血效果满意度显著高于对照组(P<0.05)。结论:Nd∶YAG激光排龈法和双线排龈法应用于前牙或前磨牙烤瓷冠修复中均能获得满意的排龈效果,但Nd∶YAG激光排龈能明显缩短患者排龈时间,降低其排龈后疼痛感,提高患者排龈止血效果满意度。Objective To explore the application effects of two-gingival retraction cords and Nd:YAG laser gingival retraction in fixed restoration of dental porcelain.Methods 120 cases(179 teeth)of anterior or premolar baked porcelain crowns were selected and randomly divided into control group(n=30 cases,92 teeth)and observation group(n=30 cases,87 teeth).Control group was given two-gingival retraction cords,and observation group was treated with Nd:YAG laser gingival retraction.The time of gingival retraction was observed in the two groups.The health status of periodontal tissues was compared between the two groups beforegingival retraction and after 1 week,1 month and 3 months of gingival retraction,and thesatisfaction after gingival retraction was investigated in the two groups.Results The time of gingival retraction in observation group was significantly shorter than that in control group(P<0.05).The improvement effects of pain level after gingival retraction in observation group was significantly better than that in control group(P<0.05).After 1 week to 3 months ofgingival retraction,the probing depth(PD)value in the two groups was decreased first and then increased,and the difference between the different time points in the group was statistically significant(P<0.05).The PD value in observation group after 1 week and 1 month of gingival retraction was significantly higher than that in control group(P<0.05).After 1 week,1 month and 3 months of gingival retraction,there were no significant differences in the results of bleeding on probing(BOP)grading and gingival index(GI)grading between the two groups(P>0.05).After gingival retraction,the satisfaction degree of gingival hemostasis in observation group was significantly higher than that in control group(P<0.05).Conclusion Nd:YAG lasergingival retraction and two-gingival retraction cords can achieve satisfactory gingival retraction effects in the restoration of anterior or premolar baked porcelain crowns,but Nd:YAG laser gingival retraction can significantly shorten the
