摘　　要：目的通过Meta分析评价异常凝血酶原(PIVKA-Ⅱ)联合甲胎蛋白(AFP)、甲胎蛋白异质体L3(AFP-L3)检测对原发性肝癌的诊断价值。方法采用互联网检索国内外数据库,根据纳入和排除标准筛选文献,评价纳入文献的质量时采用质量评价工具QUADAS-2。对所收集信息进行合并比对时使用软件Revman5.2和Meta-disc1.4。结果共纳入13篇文献,PIVKA-Ⅱ联合AFP、AFP-L3检测原发性肝癌的灵敏度、特异度、阳性似然比(PLR)、阴性似然比(NLR)、诊断比值比(DOR)分别为0.76(95%CI:0.74~0.78)、0.79(95%CI:0.77~0.81)、3.36(95%CI:2.48~5.32)、0.25(95%CI:0.18~0.35)、17.71(95%CI:9.19~34.12),综合受试者工作特征曲线的曲线下面积为0.8790。结论联合检测PIVKA-Ⅱ、AFP、AFP-L3更有助于对原发性肝癌的诊断,可作为重要的筛选指标。Objective To evaluate the diagnostic value of combination detection of abnormal prothrombin(PIVKA-Ⅱ),alpha-fetoprotein(AFP)and alpha-fetoprotein heterogeneity L3(AFP-L3)in primary hepatocellular carcinoma by Meta-analysis.Methods The Internet was used to search domestic and foreign databases,the documents were screened according to inclusion and exclusion criteria,and the quality of the included documents were evaluated with QUADAS-2.A meta-analysis was performed using Revman5.2 and Meta-disc 1.4.Results A total of 13 references were included.Sensitivity,specificity,positive likelihood ratio(PLR),negative likelihood ratio(NLR)and diagnostic odds ratio(DOR)of PIVKA-Ⅱ,AFP combined with AFP-L3 were 0.76(95%CI:0.74-0.78),0.79(95%CI:0.77-0.81),3.36(95%CI:2.48-5.32),0.25(95%CI:0.18-0.35)and 17.71(95%CI:9.19-34.12).The area under the sROC curve was 0.8790.Conclusion AFP,
