联合筋膜鞘矫正外伤性上睑下垂的临床疗效

摘　　要：目的探讨联合筋膜鞘悬吊术矫正外伤性上睑下垂的临床疗效。方法自2016年3月~2019年3月对16例(20只眼)外伤性重度上睑下垂患者应用联合筋膜鞘悬吊术进行治疗,将位于上穹隆的提上睑肌和上直肌之间的联合筋膜鞘与睑板缝合固定达到治疗上睑下垂的目的。结果术后随访3~12个月;3月随访治愈20只眼(100%);6月随访治愈19只眼(95.0%),复发1只眼(5.0%);1年随访治愈18只(90%),复发2只眼(10%)。所有患者随访期间未出现暴露性角膜炎、上睑内翻、结膜脱垂等并发症。结论联合筋膜鞘悬吊术矫正外伤性重度上睑下垂,治愈率高,并发症少,是外伤性重度上睑下垂治疗的新方法。Objective To evaluate the clinical e fficacy of combined“conjoint fascial sheath”(CFS)suspension in the treatment of traumatic blepharoptosis.Methods A total of 16 cases(20 eyes)with traumatic severe upper eyelid blepharoptosis were enrolled from March 2016 to March 2019.All the cases were treated with CFS suspension.The CFS located in the intermuscular space between the superior rectus and levator muscle of the upper eyelid was fixed to the meibomian to raise the height of the upper eyelid.Results All patients were followed up from 3 to 12 months.Among the 16 cases(20 eyes),20 eyes(100.0%)were completely cured in 3 months,19 eyes(95.0%)were completely cured and 1 eye(10.0%)was recurrence in 6 months,and 18 eyes(90.0%)were completely cured and 2 eye s(10.0%)were recurrence in 12 months.There were no exposure keratitis,upper eyelid entropion or conjunctival prolapse during the follow-up period.Conclusion CFS suspension is a new mothod for the correction of traumatic severe upper eyelid blepharoptosis.The procedure not only has highly recovery rate but also has few complications.
