摘 要:目的探讨锥形束CT在安氏错颌畸形患者进行正畸治疗后牙根吸收程度评估中的应用诊断价值。方法分别运用锥形束CT和数字化曲面体层X线摄影数字技术,检查获取2017年5月至2020年3月在我院接受正畸整形治疗的85例安氏错颌畸形患者正畸治疗前后牙列水平面、纵断面及横断面影像数据,以X线头影测量分析作为评价金标准,评价两种方法对正畸治疗前后牙根吸收程度的评估诊断准确率。结果经X线头影测量确定,正畸治疗后牙根无吸收10例,轻度吸收31例,中度吸收27例,重度吸收17例;锥形束CT诊断准确率为91.76%(78/85),明显高于数字化曲面体层X线摄影的64.71%(55/85),差异有统计学意义(x^2=12.835,P=0.004)。治疗后,锥形束CT检查显示,患者左右两侧上下侧切牙、中切牙体积均较治疗前缩小,治疗前后比较,差异有统计学意义(P<0.05)。结论正畸治疗后,错颌畸形患者牙根大多会出现不同程度的吸收,通过锥形束CT检查可准确、快速识别和诊断牙根体积变化和吸收程度,是一种安全、快捷有效的检查诊断方法。Objective To explore the diagnostic value of cone-beam CT in evaluating the degree of tooth root resorption after orthodontic treatment in patients with Angle's malocclusion.Methods Using cone beam CT and digitized tomography X-ray photography digital technology to examine and obtain the anterior and posterior teeth of 85 patients with Orthodontic Orthodontics treated in our hospital from May 2017 to March 2020 Column horizontal,longitudinal and cross-sectional image data,using X-ray cephalometric analysis as the gold standard for evaluation,to evaluate the accuracy of the two methods for evaluating the degree of root absorption before and after orthodontic treatment.Results According to X-ray cephalometric measurements,there were 10 cases of tooth root no absorption after orthodontic treatment,31 cases of mild absorption,27 cases of moderate absorption,and 17 cases of severe absorption;The diagnostic accuracy of cone-beam CT is 91.76%(78/85),which is significantly higher than that of 64.71%(55/85)of digital tomography X-ray photography.The difference is statistically significant(x^2=12.835,P=0.004).After treatment,the cone beam CT examination showed that the volume of the upper and lower incisors and middle incisors on both sides of the patient was smaller than before treatment.The di fference before and after treatment was statistically signi ficant(P<0.05).Conclusion After orthodontic treatment,most of the roots of patients with staggered jaw deformity will have different degrees of absorption.Cone beam CT examination can accurately and quickly identify and diagnose tooth root volume changes and absorption,which is a safe,fast and effective method of diagnosis.
