摘 要:目的比较口腔正畸微种植支抗(MIA)技术与传统口外弓支抗(HGA)技术联合MBT直丝弓矫治器治疗安式Ⅱ型1分类错[牙合]畸形的疗效。方法选择2015年2月至2018年2月我院收治的安式Ⅱ型1分类错[牙合]畸形患者58例,按随机对照原则分为两组,每组29例。两组均采用MBT直丝弓矫治器治疗,对照组采用HGA技术,观察组采用MIA技术。比较两组患者临床疗效、平均矫治时间、拔牙间隙关闭时间、视觉模拟评分量表(VAS)、磨牙位移、X线头影测量指标、前牙咬合情况及患者美容效果满意度。结果治疗后,观察组患者治疗总有效率、美容满意率高于对照组,差异有统计学意义(P<0.05);治疗后,观察组患者上下齿槽座角(ANB)、上中切牙长轴与前颅底平面夹角(U1-SN)、上中切牙至鼻根点与上齿槽座点连线距离(U1-NA)、下中切牙长轴与下颌平面夹角(L1-MP)、前牙覆盖(OJ)值、覆[牙合](OB)值、平均矫治时间、拔牙间隙关闭时间、VAS评分、磨牙位移均低于对照组,差异有统计学意义(P<0.05)。结论MIA技术联合MBT直丝弓矫治器治疗安式Ⅱ型1分类错[牙合]畸形能缩短疗程,改善口腔结构及头影测量指标,提高患者美容满意度。Objective To compare the effect of orthodontic micro-implant anchorage(MIA)technology and traditional external arch anchorage(HGA)technology combined with MBT straight wire arch appliance in the treatment of Angle classⅡdivision 1 malocclusion.Methods From February 2015 to February 2018,58 patients with Angle classⅡdivision 1 malocclusion who had been received in our hospital were divided into two groups according to the principle of randomized control,29 cases in each group.Both groups were treated with MBT straight wire appliance,HGA technique was used in the control group,and MIA technique was used in the observation group.The clinical efficacy,average orthodontic time,closing time between extractions,visual analogue scale(VAS),molar displacement,X-ray cephalometric measurement index,anterior teeth occlusal status,and patient’s cosmetic e ffect satisfaction were compared between the two groups.Results After treatment,the total effective rate and beauty satisfaction rate of the observation group were higher than that of the control group,the di fference was statistically signi ficant(P<0.05);After treatment,the upper and lower alveolar seat angle(ANB),the angle between the long axis of the upper central incisor and the plane of the anterior skull base(U1-SN),the connection between the upper central incisor to the root of the nose and the upper alveolar seat Distance(U1-NA),angle between the long axis of the lower central incisor and the mandibular plane(L1-MP),anterior tooth coverage(OJ)value,overbite(OB)value,average orthodontic time,tooth extraction gap closing time,VAS The score and molar displacement were lower than the control group,the di fference was statistically signi ficant(P<0.05).Conclusion MIA technique combined with MBT straight wire arch appliance for treatment of Angle classⅡdivision 1 malocclusion can shorten the course of treatment,improve oral structure and cephalometric measurement index,and improve patients’beauty satisfaction.
