食管鳞状细胞癌肿瘤细胞和间质细胞中podoplanin的表达及临床意义

摘　　要：目的探讨食管鳞状细胞癌(esophageal squamous cell carcinoma,ESCC)中podoplanin在肿瘤细胞和间质细胞的表达及其临床意义。方法应用免疫组化EnVision法检测150例ESCC中podoplanin的表达,分析肿瘤细胞、间质细胞中podoplanin表达以及两者共表达与临床病理特征之间的关系。结果150例ESCC中,肿瘤细胞podoplanin阳性者50例,其表达与淋巴结转移、脉管侵犯和TNM分期呈正相关(P<0.05);与患者性别、年龄、肿瘤大小、组织学分级、浸润深度和神经侵犯均无关(P>0.05)。podoplanin在间质细胞中的阳性率为54%(81/150),其表达与浸润深度、淋巴结转移、脉管侵犯和TNM分期均有关(P<0.05);与患者性别、年龄、肿瘤大小、组织学分级及神经侵犯无关(P>0.05)。ESCC肿瘤细胞和间质细胞中podoplanin共阳性者44例,肿瘤细胞和间质细胞中podoplanin共阳性与淋巴结转移、脉管侵犯、神经侵犯和TNM分期密切相关(P<0.05)。相关分析显示肿瘤细胞和间质细胞中podoplanin表达呈正相关(P<0.05)。结论ESCC肿瘤细胞和间质细胞中podoplanin表达可促进食管癌的发展,肿瘤细胞podoplanin表达可能诱导间质细胞表达podoplanin,两者协调促进ESCC的发生。Purpose To study the expression of podoplanin of tumor cells and stroma cells in esophageal squamous cell carcinoma(ESCC),and to explore the clinicopathological significance of podoplnin expression in different types of cells.Methods The expression of podoplanin in 150 cases of ESCC was detected by EnVision method.The relationship between the expression of podoplanin in tumor cells or stroma cells and simultaneous expression and clinicopathological parameters were analyzed.Results Among the 150 cases of ESCC,50 cases were positive for podoplanin.The expression of podoplanin in tumor cells was positively correlated with lymph node metastasis,vascular invasion and TNM stage(P<0.05),but not related to sex,age,tumor size,histological grade,depth of invasion and perineural invasion(P>0.05).The positive expression rate of podoplanin in tumor stroma cells was 54%(81/150).The expression of podoplanin in tumor stroma cells was correlated with depth of invasion,lymph node metastasis,vascular invasion,and TNM stage(P<0.05),but not correlated with sex,age,tumor size,histologic grade and perineural invasion(P>0.05).Co-expression of podoplanin in the tumor and stromal cells was seen in 44 cases of ESCC.The co-expression of podoplanin in tumor cells and stromal cells were positively related to lymph node metastasis,vascular invasion,perineural invasion and TNM stage(P<0.05).Correlation analysis showed that there was a positive correlation between the expression of podoplanin in tumor cells and stroma cells(P<0.05).Conclusion The expression of podoplanin in tumor cells and stroma cells can promote the progress of esophageal cancer in ESCC.The expression of podoplanin in tumor cells may induce the expression of podoplanin in stromal cells which promoting collaboratively the progress of ESCC.
