阿替卡因肾上腺素与利多卡因对儿童根管治疗术的麻醉效果及血流动力学的影响

摘　　要：目的:探讨阿替卡因肾上腺素与利多卡因对儿童根管治疗术的麻醉效果及血流动力学的影响.方法:回顾性分析2017年6月-2019年6月本院65例行根管治疗术患儿的临床资料,根据麻醉药物不同分为观察组(n=35)和对照组(n=30).观察组给予阿替卡因肾上腺素麻醉,对照组给予利多卡因麻醉.比较两组麻醉效果、麻醉起效时间、麻醉恢复时间及不良反应发生情况.比较麻醉前(T0)、麻醉后10 min(T1)及麻醉后20 min(T2)两组视觉模拟评分(VAS)、收缩压(SBP)、舒张压(DBP)及心率(HR).结果:观察组麻醉有效率高于对照组(P<0.05).观察组T1与T2时的VAS评分均低于对照组(P<0.05).观察组麻醉起效时间及麻醉恢复时间均短于对照组(P<0.05).两组T0、T1、T2时的SBP、DBP与HR比较,差异均无统计学意义(P>0.05).观察组不良反应发生率低于对照组(P<0.05).结论:与利多卡因比较,在儿童根管治疗术中阿替卡因肾上腺素麻醉效果更好,不良反应发生率更低,且对血流动力学无显著影响.Objective:To explore the effect of Articaine Adrenaline and Lidocaine on anesthesia and hemodynamics of root canal therapy in children.Method:The clinical data of 65 children underwent root canal therapy in our hospital from June 2017 to June 2019 were retrospectively analyzed.According to different anesthesia drugs,they were divided into observation group(n=35)and control group(n=30).The observation group was anesthetized with Articaine Adrenaline and the control group was anesthetized with Lidocaine.The success rate of anaesthesia,the time of anaesthesia onset,the time of anaesthesia recovery and the occurrence of adverse reactions were compared between the two groups.Visual analogue scale(VAS),systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)were compared between the two groups before anesthesia(T0),10 min after anesthesia(T1)and 20 min after anesthesia(T2).Result:The effective rate of anesthesia in the observation group was higher than that in the control group(P<0.05).VAS scores at T1 and T2 in the observation group were lower than those in the control group(P<0.05).The onset time and recovery time of anesthesia in the observation group were lower than those in the control group(P<0.05).There were no significant differences on SBP,DBP and HR at T0,T1 and T2 between the two groups(P>0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion:Compared with Lidocaine,the anesthetic effect of Articaine Adrenaline is better,the incidence of adverse reactions is lower,and there is no significant effect on hemodynamics.
