血清CYFRA21-1、CA199和SCC对食管癌的诊断与治疗效果评估价值

摘　　要：目的:探讨血清肿瘤相关细胞角蛋白19片段(CYFRA21-1)、糖链抗原199(CA199)和鳞状细胞癌抗原(SCC)对食管癌的诊断与治疗效果评估价值.方法:回顾性分析2016年1月-2019年10月在本院进行治疗的食管癌患者80例(病例组)及同时段的健康体检者80例(对照组)的资料,分析两组血清CYFRA21-1、CA199和SCC的浓度.结果:病例组血清CYFRA21-1、CA199和SCC的浓度均高于对照组,差异均有统计学意义(P<0.05).不同病理分期患者血清CYFRA21-1、CA199和SCC浓度比较,差异均有统计学意义(P<0.05).Ⅱ、Ⅲ、Ⅳ期患者血清CYFRA21-1、CA199和SCC浓度均高于Ⅰ期患者,Ⅲ、Ⅳ期患者血清CYFRA21-1、CA199和SCC浓度均高于Ⅱ期患者,Ⅳ期患者血清CYFRA21-1、CA199和SCC的浓度均高于Ⅲ期患者,差异均有统计学意义(P<0.05).使用血清CYFRA21-1、CA199和SCC的浓度对食管癌进行诊断,最佳截断值分别为2.88 ng/mL、68.80 U/mL和1.23μg/L,约登指数分别为76.25%、58.75% 和72.50%.治疗后,病例组血清CYFRA21-1、CA199和SCC的浓度均低于治疗前,差异均有统计学意义(P<0.05).结论:血清CYFRA21-1、CA199和SCC浓度可以作为食管癌诊断与治疗效果的参考指标,但是需要进行大规模人群试验的验证.Objective:To explore the evaluation value of serum tumor-associated cytokeratin 19 fragment(CYFRA21-1),carbohydrate antigen 199(CA199)and squamous cell carcinoma antigen(SCC)in the diagnosis and treatment of esophageal carcinoma.Method:The data of 80 patients(the case group)with esophageal cancer treated in the hospital from January 2016 to October 2019 and 80 healthy persons(the control group)in the same period were analyzed retrospectively.The serum concentrations of CYFRA21-1,CA199 and SCC in two groups were analyzed.Result:The serum concentrations of CYFRA21-1,CA199 and SCC in the case group were all higher than those in the control group,the differences were statistically significant(P<0.05).The serum CYFRA21-1,CA199 and SCC in patients with different pathological stages compared,the differences were statistically significant(P<0.05).The concentrations of CYFRA21-1,CA199 and SCC in patients with stageⅡ,ⅢandⅣwere higher than those in patients with stageⅠ,the concentrations of CYFRA21-1,CA199 and SCC in patients with stageⅢandⅣwere higher than those in patients with stageⅡ,and the concentrations of CYFRA21-1,CA199 and SCC in patients with stageⅣwere higher than those in patients with stageⅢ,the differences were statistically significant(P<0.05).For the diagnosis of esophageal cancer,the optimal cut-off values of serum CYFRA21-1,CA199 and SCC were 2.88 ng/mL,68.80 U/mL and 1.23μg/L,respectively,Youden index were 76.25%,58.75%and 72.50%.After treatment,the serum concentrations of CYFRA21-1,CA199 and SCC in the case group were all lower than those of before treatment,the differences were statistically significant(P<0.05).Conclusion:Serum concentrations of CYFRA21-1,CA199 and SCC can be used as reference indexes for the diagnosis and treatment of esophageal cancer,but need to be verified by large-scale population trials.
