宫颈连续环形缝合术在宫颈冷刀锥切术中的应用研究

摘　　要：目的:探讨宫颈冷刀锥切术后连续环形缝合法的临床应用价值.方法:选取2016年10月-2019年4月在本院妇科诊断为宫颈高级别病变(HSIL),行宫颈冷刀锥切术的患者61例为研究对象.随机分为连续环形缝合组(30例)和传统"W"型缝合组(31例).比较两组的手术时间、锥高、锥底直径、术中出血量、术后出血量、术后住院时间及宫颈黏膜增生情况.结果:两组锥高、锥底直径、术后出血量、手术时间、术后住院时间比较,差异均无统计学意义(P>0.05);连续环形缝合组术中出血量少于传统"W"型缝合组,差异有统计学意义(P<0.05).传统"W"缝合组术后出现4例宫颈黏膜增生,连续环形缝合组未出现宫颈黏膜增生,两组比较差异有统计学意义(P<0.05).两组术后均未采取医疗措施止血,均未出现宫颈粘连或狭窄.结论:宫颈连续环形缝合法术中止血效果确切,术后宫颈成形美观,值得临床推广应用.Objective:To explore the clinical application value of continuous circular suture after cervical cold knife conization.Method:A total of 61 patients who diagnosed with high grade cervical lesion(HSIL),in the department of gynaecology of our hospital from October 2016 to April 2019 and underwent cervical cold knife conization were selected as the study subjects.They were randomly divided into the continuous circular suture group(30 cases)and the traditional"W"suture group(31 cases).The operative time,cone height,cone bottom diameter,intraoperative blood loss,postoperative blood loss,postoperative hospital stay and cervical mucosal hyperplasia were compared between two groups.Result:There were no significant difference between two groups in cone height,cone bottom diameter,postoperative blood loss,operation time and postoperative hospital stay(P>0.05).The intraoperative blood loss in the continuous circular suture group was less than that in the traditional"W"suture group,the difference was statistically significant(P<0.05).There were 4 cases of cervical mucosal hyperplasia in the traditional"W"suture group,but no cervical mucosal hyperplasia in the continuous circular suture group,the difference between two groups was statistically significant(P<0.05).In two groups,no medical measures were taken to stop bleeding,and no cervical adhesion or stenosis were observed.Conclusion:The hemostatic effect of continuous circular suture of cervix is accurate and the cervix is beautiful after operation,which is worthy of clinical application.
