整层充填流动树脂一次固化充填与传统树脂分层充填治疗牙齿楔状缺损的效果分析

摘　　要：目的观察整层充填流动树脂一次固化充填与传统树脂分层充填对牙齿楔状缺损的治疗效果。方法选取2015年8月至2016年8月接受治疗的双侧牙齿楔状缺损的58例患者为研究对象,左侧为观察组,右侧为对照组。观察组给予整层充填流动树脂一次固化充填,对照组给予传统树脂分层充填,充填后1周、1个月、1年、3年复诊,使用改良美国公共卫生署评价系统(USPHS标准)评价2组患者的治疗效果,比较2组患者的术后敏感发生率和树脂充填的不良结局(包括外形完整度、边缘着色、继发龋)发生率,并在3年复诊时评价树脂充填满意度。结果观察组患者治疗树脂充填术后1周、1个月、1年的敏感发生率均明显低于观察组(χ2值=7.817、5.327、3.867,P值=0.005、0.021、0.049),术后3年复诊患牙外形完整度、边缘着色、继发龋的不良结局发生率观察组明显低于对照组(χ2值=5.199、3.940、4.245,P值=0.023、0.047、0.039);观察组3年满意率为96.55%明显高于对照组82.76%,差异有统计学意义(χ2值=5.983,P值=0.014))。结论整层充填流动树脂一次固化充填在牙齿楔状缺损的治疗效果良好,术后短期敏感发生率低,长期不良结局发生率低,具有良好的应用价值。Objective To investigate the therapeutic effects between one time whole layer filling of flow resin and traditional resin stratified filling in treatment of wedge-shaped defect of tooth.Methods A total of 58 patients with both sides wedge-shaped defect of tooth who were treated in our hospital from August 2015 to August 2016 were enrolled in the study.The patients with left side wedge-shaped defect of tooth were enrolled as observation group,and the patients with right side wedge-shaped defect of tooth were enrolled as control group.The patients in observation group were treated by one time whole layer filling of flow resin,however,the patients in control group were treated by traditional resin stratified filling.The patients were followed up at 1w,1m,1y,3y after the filling.The therapeutic effects were evaluated according to the modified USPHS criteria.The incidence rates of postoperative sensitivity and bad outcomes including filling shape,edge coloring,secondary caries were observed and compared between the two groups.After 3-year follow up,the treatment satisfaction degree was evaluated and compared between the two grousp.Results The incidence rates of postoperative sensitivity at 1w,1m,1y,3y after the filling in observation group were significantly lower than those in control group(P<0.01 or P<0.05).The incidence rates of bad outcomes including filling shape,edge coloring,secondary caries after 3-year follow up were significantly lower than those in control group(P<0.05).The satisfaction rate in observation group was 96.55%,which was significantly higher than that(82.76%)in control group(P<0.05).Conclusion The one time whole layer filling of flow resin has better therapeutic effects in treatment of wedge-shaped defect of tooth,which can reduce the incidence rates of postoperative sensitivity and bad outcomes of patients,therefor,which has better application value in clinical practice.
