摘 要:目的通过对河北省综合三甲医院护士的共情疲劳现状调查,探寻其影响因素,为改善护士共情疲劳提供理论依据。方法采用一般资料问卷、职业认知状况问卷、专业生活质量量表、心理复原力量表、社会支持评定量表,对河北省11家综合三甲医院的4913名护士进行问卷调查,分析护士共情疲劳的影响因素。结果河北省综合三甲医院护士的共情满意得分为(29.92±8.116)分,职业倦怠得分为(30.48±3.271)分,继发性创伤应激得分为(31.65±8.487)分。最高学历、心理复原力、社会支持等是共情满意得分的影响因素(P<0.05);选择护理专业的原因、工作中是否缺乏团队协作等是职业倦怠的影响因素(P<0.05);职称、聘任方式、所在科室、心理复原力、社会支持等是继发性创伤应激的影响因素(P<0.05)。心理复原力与河北省综合三甲医院护士的共情满意呈正相关(r=0.578,P<0.01),与护士的职业倦怠呈负相关(r=-0.109,P<0.01),与护士的继发性创伤应激呈负相关(r=-0.758,P<0.01);社会支持与护士的共情满意呈正相关(r=0.357,P<0.01),与护士的职业倦怠呈负相关(r=-0.179,P<0.01),与护士的继发性创伤应激呈负相关(r=-0.596,P<0.01)。结论河北省综合三甲医院护士处于重度共情疲劳;心理复原力差,社会支持小的护士更容易发生共情疲劳,护理管理者应该多关注此类人员。Objective To investigate the the current situation of empathy fatigue and influencing factors in nurses in comprehensive grade-A tertiary hospital in Hebei province,so as to provide theoretical basis for improving nurses’empathy fatigue.Methods A general questionnaire,occupational cognitive status questionnaire,professional quality of life scale,psychological recovery strength table,and social support rating scale were used to perform a questionnaire survey in 4,913 nurses in 11 comprehensive grade-A tertiary hospital in Hebei province to analyze the influencing factors on empathy fatigue.Results The empathy satisfaction scores of nurses in comprehensive grade-A tertiary hospital in Hebei province were(29.92±8.116)points,the occupation tireness scores were(30.48±3.271)points,and the secondary traumatic stress scores were(31.65±8.487)points.The highest educational background psychological resilience,social support were the influencing factors on empathy satisfaction score(P<0.05).and the reasons for choosing nursing major,lack of team cooperation in work were the influencing factors on occupation tireness,and job title,appointment method,department,psychological resilience and social support were the influencing factors on secondary traumatic stress(P<0.05).Moreover the psychological recovery strength was positively correlated to empathy satisfaction degree(P<0.01),which was negatively correlated to occupation tireness(P<0.01),which was negatively correlated to secondary traumatic stress(P<0.01).In addition the social support was positively correlated to empathy satisfaction degree(P<0.01),which was negatively correlated to occupation tireness and secondary traumatic stress(P<0.01).Conclusion The nurses in comprehensive grade-A tertiary hospital in Hebei province are in severe empathy fatigue status,their psychological resilience is poor,the nurses with small social support are more likely to suffer from empathy fatigue,and nursing managers should pay much attention to such personnel.
