复方苦参注射液对晚期胃癌化疗患者临床效果及CXCR4和SDF-1蛋白水平的影响

摘　　要：目的研究复方苦参注射液对晚期胃癌化疗患者临床效果及趋化因子受体4(CXCR4)和基质细胞衍生因子1(SDF-1)蛋白水平的影响。方法选取从2017年5月~2018年12月我院收治的200例晚期胃癌患者进行研究,将其以随机抽签法均分成试验组和对照组。对照组予以奥沙利铂、亚叶酸钙与5-氟尿嘧啶常规化疗治疗,试验组为奥沙利铂、亚叶酸钙与复方苦参注射液治疗。分别比较两组在近期疗效,治疗前后CXCR4和SDF-1蛋白水平,治疗前后各项免疫功能指标水平,不良反应发生情况等方面的差异。结果试验组总有效率相比对照组较高(P<0.05)。治疗后试验组CXCR4和SDF-1蛋白阳性率相比对照组较低(均P<0.05)。治疗后试验组CD3+、CD4+、CD4+/CD8+水平相比对照组较高,而CD8+水平相比对照组较低(均P<0.05)。试验组恶心呕吐、血小板减少、白细胞减少、周围神经毒性发生率相比对照组较低(均P<0.05)。结论相较于5-氟尿嘧啶,复方苦参注射液有效提高晚期胃癌化疗患者临床效果,同时有利于下调CXCR4和SDF-1蛋白水平,增强机体免疫功能,降低不良反应发生风险。Objective To investigate the clinical effect of Compound radix sophorea flavescentis injection on chemotherapy of advanced gastric cancer patients and the effects of chemokine receptor 4(CXCR4)and stromal cell-derived factor 1(SDF-1)protein levels.Methods 200 patients with advanced gastric cancer admitted to our hospital from May 2017 to December 2018 were selected for study,and they were divided into experimental group and control group by random lottery.The control group was treated with L-OHP,Calcium folate,5-FU while the experimental group was treated with L-OHP,Calcium folate,compound radix sophorea flavescentis injection.The short-term efficacy,CXCR4 and SDF-1 protein levels before and after treatment,the levels of various immune function indicators before and after treatment,and the incidence of adverse reactions of the two groups were compared.Results The total effective rate of the experimental group was higher than the control group(P<0.05).After treatment,the positive rates of CXCR4 and SDF-1 protein in the experimental group were lower than the control group(all P<0.05).After treatment,the levels of CD3+,CD4+,CD4+/CD8+in the experimental group were higher than the control group,while the levels of CD8+were lower than the control group(all P<0.05).The incidence of nausea,vomiting,thrombocytopenia,leukopenia and peripheral neurotoxicity in the experimental group was lower than the control group(all P<0.05).Conclusion Compared to 5-FU,compound radix sophorea flavescentis injection can effectively improve the clinical effect of chemotherapy for advanced gastric cancer patients,and at the same time is conducive to down-regulating CXCR4 and SDF-1 protein levels,enhancing the immune function of the body,and reducing the risk of adverse reactions.
