摘  要:目的研究3D-Slicer软件辅助下神经内镜与显微镜下微血管减压术治疗原发性面肌痉挛的疗效和安全性。方法分析2015年11月-2018年11月43例原发性面肌痉挛病人的临床资料,根据手术方式随机分为内镜组(n=24)和对照组(n=19),两组病人术前均采用3D-Slicer软件将多模态MRI影像配准融合并重建出三维虚拟现实模型,内镜组病人行完全神经内镜下微血管减压术,开颅组行传统显微镜下微血管减压术,比较两组的治愈率、复发率及术后并发症的发生率。结果两组对比,患者的治愈率无统计学意义(P>0.05),而内镜组的并发症发生率及复发率均低于对照组(P<0.05)。结论3D-Slicer软件辅助神经内镜及显微镜下微血管减压术均能有效的治疗原发性面肌痉挛,而神经内镜辅助下微血管减压术具有减少并发症的发生率及复发率等优势,是治疗原发性面肌痉挛的一种更为安全可靠的手术方式,可能成为原发性面肌痉挛更有前途的治疗方法。Objective To study the efficacy and safety of 3 D-Slicer software assisted neuroendoscopy and microscope microvascular decompression in the treatment of primary hemifacial spasm.Methods The clinical data of 43 patients with primary hemifacial spasm from November 2015 to November 2018 in our hospital were retrospectively analyzed.According to the surgical methods,43 patients were randomly divided into two groups:endoscopic group(n=24)and control group(n=19).The multimodal MRI images were fused with 3 D-Slicer software before operation and the 3 D virtual reality model was reconstructed.The patients in the endoscopic group underwent complete endoscopic microvascular decompression and the craniotomy group underwent the traditional microvascular decompression under microscope.The cure rate,recurrence rate and postoperative complication rate were compared between the two groups.Results There was no significant difference in the cure rate between the two groups(P>0.05).The incidence of complications and the recurrence rate in the endoscopic group were lower than those in the control group(P<0.05).Conclusions 3 D-Slicer software assisted neuroendoscopy and microvascular decompression under microscope can effectively treat primary hemifacial spasm,while neuroendoscopy-assisted microvascular decompression has the advantages of reducing the incidence of complications and recurrence rate.It is a more safe and reliable surgical method for primary hemifacial spasm and may be a more promising treatment for primary hemifacial spasm.
