摘 要:目的探讨不同病理类型肺腺癌增殖细胞核抗原Ki-67水平变化与多层螺旋CT(MSCT)征象特征的相关性。方法收集经手术病理证实的肺腺癌患者142例,采用Goodman-Kruskal Gamma方法分析术后不同病理类型肺腺癌组织的Ki-67表达情况,并采用Spearman相关分析Ki-67表达与MSCT征象的相关性。结果肺腺癌组织中Ki-67表达程度与肿瘤的恶性程度[原位腺癌(AIS)-微浸润腺癌(MIA)-浸润腺癌(IAC)]呈正相关(G=0.976,P<0.01)。Ki-67的表达程度与MSCT征象(密度、直径、分叶征、毛刺征)均呈高度或中度正相关(r=0.734、0.664、0.539、0.521,均P<0.01),而与胸膜凹陷征呈弱正相关(r=0.3,P<0.05),与空泡征无相关性(r=0.142,P>0.05)。结论肺腺癌的恶性程度与Ki-67表达呈正相关,同时肿瘤的MSCT征象与Ki-67表达有一定相关性,综合分析肺肿瘤MSCT影像学特征,可对Ki-67表达水平具有潜在预测价值。Objective To investigate the correlation between the expression of proliferating cell nuclear antigen Ki-67 and multi-slice CT(MSCT)features in different types of lung adenocarcinoma.Methods One hundred and forty-two patients with lung adenocarcinoma confirmed by operation and pathology in our hospital from January 2017 to June 2018 were enrolled.The expression of Ki-67 in cancer tissue samples was analyzed by Goodman-Kruskal Gamma method,and the correlation between the expression of Ki-67 and MSCT signs was analyzed by Spearman correlation method.Results There was a significant positive correlation between the expression of Ki-67 and malignant degree of lung adenocarcinoma(AIS-MIA-IAC)(G=0.976,P<0.01).The expression of Ki-67 was correlated with tumor density and diameter,lobulation and burr signs in MSCT images(r=0.734,0.664,0.539 and 0.521,respectively,P<0.01),while there was no correlation with vacuole(r=0.142,P>0.05).Conclusion The malignant degree of lung adenocarcinoma is positively correlated with the expression of Ki-67,and the MSCT signs of lung adenocarcinoma are correlated with the expression of Ki-67.Comprehensive analysis of MSCT imaging features of lung adenocarcinoma can potentially predict the expression level of Ki-67.
