摘  要:目的分析浙江省丽水地区人群亚甲基四氢叶酸还原酶(MTHFR)基因C677T和A1298C单核苷酸多态性与人乳头瘤病毒(16、18、52、58)感染的宫颈鳞状细胞癌发病风险的关系。方法采用实时荧光聚合酶链式反应方法检测在丽水市中心医院住院治疗的115例HPV(16、18、52、58)感染的宫颈鳞状细胞癌患者和143例HPV阴性的健康对照妇女血液标本MTHFR C677T和A1298C两个单核苷酸多态性基因型频率的分布,分析其与HPV(16、18、52、58)感染宫颈鳞状细胞癌发病风险的关系;28种HPV基因型采用聚合酶链式反应-反向点杂交法对宫颈分泌物标本进行检测。结果病例组和对照组MTHFR C677T和A1298C基因型频率分布均符合Hardy-Weinberg平衡定律(均P>0.05),病例组和对照组MTHFR C677T和A1298C基因型频率分布均有统计学差异(均P<0.05)。在MTHFR C677T多态性位点中,携带T的受试者HPV(16、18、52、58)感染的宫颈鳞状细胞癌发病风险是C受试者的1.871倍(OR=1.871,95%CI:1.294~2.705,P<0.05);在MTHFR A1298C多态性位点中,携带C的受试者HPV(16、18、52、58)感染宫颈鳞状细胞癌发病风险是A受试者的1.867倍(OR=1.867,95%CI:1.204~2.896,P<0.05)。结论MTHFR C677T和A1298C多态性可能与浙江省丽水地区人群HPV(16、18、52、58)感染者宫颈鳞状细胞癌发病有关。Objective To investigate the relationship between the single nucleotide polymorphisms of methylenetetrahy-drofolate reductase(MTHFR)genes(C677T and A1298C)and the risk of human squamous cell carcinoma infected by human papillomavirus(16,18,52,58).Methods One hundred and fifteen patients with HPV(16,18,52,58)infected cervical squamous cell carcinoma admitted to Lishui Central Hospital and 143 HPV-negative healthy female subjects were enrolled in the study.The frequency distribution of MTHFR C677T and A1298C SNP genotypes in blood samples from all subjects were detected by real-time fluorescent polymerase chain reaction,and the relationship of gene polymorphism with HPV(16,18,52,58)infection and cervical squamous cell carcinoma was analyzed.Polymerase chain reaction-reverse dot hybridization was used to detect 28 human papillomavirus genotypes in cervical secretion specimens.Results The frequency distributions of MTHFR C677T and A1298C genotypes in the case group and control group were in accordance with Hardy-Weinberg equilibrium law(P>0.05).The frequency distributions of genotypes and alleles of MTHFR C677T and A1298C in the case group and control group were significantly different(both P<0.05).The risk of HPV(16,18,52,58)-infected cervical squamous cell carcinoma in individuals carrying the MTHFR C677TT was 1.871 time higher than individuals carrying C(OR=1.871,95%CI:1.294-2.705,P<0.05);in the MTHFR A1298C polymorphism site,the risk of HPV(16,18,52,58)-infected cervical squamous cell carcinoma in individuals carrying C was 1.867-fold higher than that in individuals carrying A(OR=1.867,95%CI:1.204-2.896,P<0.05).Conclusion The MTHFR C677T and A1298C polymorphisms may be associated with the risk of cervical squamous cell carcinoma infection by human papillomavirus(16,18,52,58).
