方体定向原理指导下经额侧脑室前角穿刺精准性和安全性初步探讨

摘　　要：目的探讨方体定向原理指导下经额侧脑室前角穿刺的精准性和安全性。方法前瞻性纳入2017年1月~2019年1月盐城市第一人民医院急诊科收治的脑出血患者共48例,所有患者均采用孙树杰方体定向原理指导下经额改良侧脑室前角穿刺。结果1次穿刺成功率为100%,其中45例在同侧侧脑室前角,3例在对侧侧脑室前角;平均穿刺置管深度为7.0±0.3cm,偏开角为0°~10°;穿刺道周围出血发生率为0;癫痫发生率为0。结论方体定向原理指导下经额改良侧脑室前角穿刺是一种安全有效、简单方便的脑室穿刺方法,尤其适用于狭小脑室,值得推荐使用。Objective To investigate the accuracy and safety of transfrontal puncture of anterior horn of lateral ventricle guided by the principle of orientation.Methods A total of 48 patients with cerebral hemorrhage admitted to the emergency department of Yancheng City NO.1 People′s Hospital from January 2017 to January 2019 were prospectively included.All patients underwent lateral ventricle puncture.Results The success rate of the first puncture was 100%.45 cases were in the anterior horn of ipsilateral ventricular and 3 cases were in the anterior horn of contralateral ventricular.The average puncture depth was 7.0±0.3cm,tube after partial open Angle of 0°to 10°;The incidence of hemorrhage around puncture tube was 0.The incidence of epilepsy was 0.Conclusion The modified transfrontal puncture of anterior horn of lateral ventricle guided by the principle of cube orientation is a safe,effective,simple and convenient method for ventricle puncture,especially for small ventricles.
