阿加曲班联合阿司匹林对后循环脑梗死的效果评价

摘　　要：目的探讨阿加曲班联合阿司匹林对后循环脑梗死的效果。方法将2017年2月至2018年9月60例后循环脑梗死患者随机分为两组各30例,其中一组单用阿司匹林肠溶片治疗,另一组采用阿司匹林肠溶片与阿加曲班联合治疗,分别设为单用组与联合组。治疗前后分别对患者进行脑血流动力学测定、实验室指标检测及神经功能测定。结果两组治疗后的脑血管平均血流速度(V)、平均血流量(Q)、血管外周阻力(R)值均明显改善,且联合组改善情况明显优于单用组(P<0.05)。两组治疗后的血小板计数(PLT)、血小板压积(MVP)、S100B蛋白、神经元特异性烯醇化酶(NSE)水平均明显改善,且联合组改善情况明显优于单用组(P<0.05)。两组治疗后1周、2周、4周、8周的NIHSS评分均明显降低(P<0.05),联合组明显低于单用组(P<0.05)。结论阿加曲班联合阿司匹林可有效改善脑血流动力学,改善神经功能,对后循环脑梗死具有不错的效果。Objective To investigate the effect of acataban on posterior circulation cerebral infarction.Methods 60 patients with posterior circulation cerebral infarction from February 2017 to September 2018 were randomly divided into two groups,30 cases in each group.One group was treated with aspirin enteric-coated tablets alone,and the other group was treated with aspirin enteric-coated tablets in combination with agartriban,which were set as control group and agartriban group,respectively.Before and after treatment,cerebral blood flow dynamics,laboratory indexes and nerve function were measured.Results After treatment,the average velocity of cerebral blood flow(V),average volume of blood flow(Q)and peripheral obstruction force(R)of blood vessels in both groups were significantly improved,and the improvement in the group of agartriban were significantly better than that in the control group(P<0.05).The levels of platelet count(PLT),platelet thrombosis(MVP),S100B protein and neuron specific enolase(NSE)were significantly improved in the two groups after treatment,and the improvement in the group of agartriband were significantly better than that in the control group(P<0.05).The NIHSS scores of the two groups at 1,2,4 and 8 weeks after treatment were significantly decreased(P<0.05),and the scores of the group with agartriban were significantly lower than those of the control group(P<0.05).Conclusion Agastroban can effectively improve cerebral blood flow dynamics,improve neurological function,and has a good effect on posterior circulation cerebral infarction.
