戊己清肠方保留灌肠治疗湿热夹瘀型腹泻型肠易激综合征(IBS-D)疗效观察

摘　　要：目的:观察戊己清肠方保留灌肠治疗湿热夹瘀型腹泻型肠易激综合征(IBS-D)效果。方法:选取2017年03月~2019年03月我院收治的100例湿热夹瘀型IBS-D患者为对象,采用随机数字法分为对照组(n=50)和观察组(n=50),对照组给予匹维溴铵治疗,观察组给予戊己清肠方保留灌肠治疗,比较两组临床疗效、中医症候积分、血清炎症因子[白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]、血清5-羟色胺(5-HT)、胃动素(MOT)水平、免疫功能及不良反应发生率。结果:治疗后观察组总有效率为90.00%显著高于对照组的72.00%(P<0.05);治疗后观察组中医症候积分低于对照组(P<0.05);治疗后观察组血清IL-8、TNF-α、5-HT、MOT水平均显著低于对照组(P<0.05);治疗后观察组CD3+、CD4+、CD4+/CD8+均显著高于对照组,CD8+低于对照组(P<0.05);两组不良反应发生率比较无统计学意义(P>0.05)。结论:戊己清肠方保留灌肠治疗湿热夹瘀型IBS-D效果显著,能够缓解胃肠道症状,降低血清炎性因子,提高免疫功能。Objective:To observe the curative effect of Wuji Qingchang Recipe retention enema on irritable bowel syndrome(IBS-D)of damp-heat type.Methods:100 patients with IBS-D of damp-heat type who were admitted to the hospital from March 2017 to March 2019 were enrolled.They were divided into controlled group(n=50)and observation group(n=50)by random number table method.The controlled group was treated with pinaverium bromide,while the observation group was given Wuji Qingchang Recipe retention enema.The clinical curative effect,TCM symptoms scores,levels of serum inflammatory factors[interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)],serum 5-serotonin(5-HT)and motilin(MOT),immune function,and incidence of adverse reactions were compared between the two groups.Results:After treatment,the total response rate of the observation group was significantly higher than that of the controlled group(90.00%vs 72.00%).After treatment,TCM syndromes scores of the observation group were lower than those of the controlled group(P<0.05).After treatment,levels of serum IL-8,TNF-α,5-HT and MOT in the observation group were significantly lower than those in the controlled group(P<0.05).After treatment,CD3+,CD4+and CD4+/CD8+in the observation group were significantly higher than those in the controlled group,while CD8+was lower than that in the controlled group(P<0.05).There was no significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion:The curative effect of Wuji Qingchang Recipe retention enema is significant on IBS-D of damp-heat type,which can relieve gastrointestinal symptoms,reduce serum inflammatory factors,and improve immune function.
