龙蛭汤对急性脑梗死患者神经缺损功能及血液流变学影响

摘　　要：目的:观察龙蛭汤对急性脑梗死(气虚血瘀型)患者神经缺损功能及血液流变学影响。方法:选取2017年1月~2018年10月我院中医科收治的120例急性脑梗死患者,按随机数字表法分为A组(60例)和B组(60例),A组采取常规西医治疗,B组在此基础上加用龙蛭汤,并比较两组患者治疗效果,治疗前后神经功能变化、神经功能相关因子、血液流变学指标以及治疗期间不良反应发生率。结果:与A组比较,B组患者治疗有效率明显升高(P<0.05);B组和A组患者治疗后美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力量表(ADL)评分、神经元特异性烯醇化酶(NSE)、血清髓鞘碱性蛋白(MBP)、胰岛素样生长因子-1(IGF-1)、红细胞压积(Packed cell volume,PCV)、全血粘度(Whole blood viscosity,ηb)和纤维蛋白(Fibrin,Fib)均与治疗前比较,差异有统计学意义(P<0.05),B组患者NIHSS评分、NSE、MBP、PCV、ηb和Fib水平均明显低于A组(P<0.05),而ADL评分和IGF-1则明显高于A组(P<0.05);两组患者均未发生严重不良反应,不良反应发生率比较,差异无统计学意义(P>0.05)。结论:龙蛭汤对急性脑梗死具有较高的治疗效果,不仅能有效改善血液流变学,同时还能促进患者神经功能恢复。Objective:To explore the effect of Longzhi Decoction on nerve function impairment and hemorheology for patients with acute cerebral infarction.Methods:120 patients with acute cerebral infarction admitted to our hospital from January 2017 to October 2018 were randomly divided into group A(60 cases)and group B(60 cases),the group A was treated with routine western medicine,and the group B was treated with Longzhi Decoction on the basis of routine western medicine,the therapeutic effect,changes of nerve function,hemorheological indexes and incidence of adverse reactions were compared between different groups before and after treatment.Results:Compared with the group A,the effective rate of treatment in the group B was significantly higher(P<0.05).There were significant differences in NIHSS score,NSE,MBP,PCV,ηb and Fib levels in the group B and the group A before and after treatment(P<0.05),and the NIHSS score,NSE,MBP,PCV,ηb and Fib levels in the group B were significantly lower than those in the group A(P<0.05),while the ADL score and IGF-1 levels in the group B were significantly higher than those in the group A.There were no serious adverse reactions in both groups,and there was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conlusion:Longzhi Decoction has a high therapeutic effect on acute cerebral infarction,can effectively improve hemorheology,and can also promote the recovery of patients’nervous function.
